Test the rise of psychological research is embodied in the field of psychology research into the exam, followed by middle school students’ test anxiety and test selfefficacy self-efficacy problems is highly concern two aspects of the study group. At the same time, the physical activity of middle school students, especially high school students is now a major subject of health psychology. Existing a lot of research has shown that high school students in a state of meditation for a long time, lead to the lack of physical activity, which not only affect the healthy body, also affects the psychological problems, so increase awareness of sports events for high school students are also gradually improve.To supplement the sports activities in the psychological test the research direction of vacancy, rich sports activities with psychological exam group has practical effect of empirical basis. This study chooses ri-chang zheng diagnosis test anxiety scale, maqing compiled test self-efficacy questionnaire and the WHO promote the international physical activity questionnaire long investigation of yunnan normal university affiliated century jin yuan all school grade three students examination anxiety, self-efficacy and the basic situation of physical activity, analysis the relationship among; Using literature method, content analysis method, panel discussion method and experience summary method, collecting, sorting and summarizing the expert counselling instance, sum up the experiences of predecessors’ research, preliminary design and preparation of the "physical activity combined with test psychological counseling handbook". Design, and through the experimental group- control group before and after the test for eight weeks, 1 times a week, about 80 minutes of group psychological counseling, the use effect of validation manual.This study the following conclusions:(1) Examination anxiety and general selfefficacy has significant negative correlation relationship; Test anxiety and the amount of physical activity was significantly negative correlation; Self-efficacy and the amount of physical activity was significantly positive correlation. 20% of the high three students in a moderate level of anxiety, 15.88% of students examination self-efficacy is low. Girls have higher test anxiety and the test lower self-efficacy; Science students have higher test anxiety, single exam self-efficacy is relatively higher; Not only children test anxiety is higher, lower self-efficacy in the exam. High three students overall physical activity levels in the medium, 10% of the students of senior three physical activity is low, science not only children less girl’s physical activity.(2) Develop the application of physical activity combined with psychological exam way of group guidance for senior students examination anxiety and self-efficacy to improve the sports activities with psychological exam group guidance manual.(3) The use of the sports activities with psychological exam group guidance manual for high three student counseling intervention, the results show that the study can effectively reduce the level of test anxiety, improve the level of test self-efficacy, and can improve test scores, student’s satisfaction.
