This article firstly gave a general introduction to the definition, forming process, effects on students, reasons and counseling strategies of test anxiety and then based on various relative resources from China and foreign countries and on a general survey on teachers and students, it made a questionnaire of test anxiety which has a profound credibility. According to the model of the questionnaire, all the testers are classified into three groups. Secondly, the article made a survey using this questionnaire on 727 high-school students. The result shows that (1) the ratio of the students who have test anxiety in high school is relatively high, about 15.7%; (2) the developmental characteristic of test anxiety among junior high school students is low-high-low; while the developmental characteristic of test anxiety among senior high school students is high to low; so is the characteristic of test anxiety among female and male students and among common high school students; but the characteristic of test anxiety among key high school students is different, which is low to high; (3) there is a significant difference of test anxiety between different grades and sexes, but no significant difference between different schools; (4) the key reason of test anxiety is students cognitive evaluation of tests, and the main reasons are family education, school education and social surroundings.Finally, the article gave a deeper examination on the relations between high school students' test anxiety and academic performance and mental health. The results show that (1) there is a significant negative relation between test anxiety and academic performance; (2) generally there is a significant negative relation between test anxiety and mental health; except the feelings of happiness, responsibility and independence, there is a significant negative relation between test anxiety and other factors of mental health; (3) generally, there is a positive relation between mental health and academic performance; except cognitive awareness, slight excitement, feeling of happiness, extraversion, soul identical and human relations, there is a significant positive relation between mental health and academic performance.
