Objective: Viral encephalitis(VE) is caused by Virus invade the centralnervous system to induce the brain parenchyma and meningeal inflammation.The virus has extremely simple life forms in biological community structure,and rely entirely on the host cell's energy and metabolic systems, access to thematerial and energy for life activities in order to maintain the organismcharacteristics and invasiveness. After invading the central nervous system,virus often leads to inflammation, edema, necrosis of the nerve cell changes,thereby resulting in the injury of the brain. In recent years, neuron specificenolase(NSE), as biochemical markers of brain damage, cause the greatattention of people, and become one of the hotspots in the research. Thisarticle, firstly, by measuring the NSE levels in cerebrospinal fluid of patientswith viral encephalitis between acute and recovery stage, intends to explorethe value in Condition monitoring and prognostic evaluation for viralencephalitis; secondly, this paper also set up other central nervous systeminfection controls what are difficult to identify with viral encephalitis inclinical early. Although tuberculous meningitis, purulent meningitis and viralencephalitis have many similar characteristics in the clinical performance early,through observation abnormalities of the NSE in them cerebrospinal fluid, weintend to explore the differential diagnosis significance for NSE between thethree common infections of central nervous system disease in clinical early.Methods: We collected90patients hospitalized in Hebei MedicalUniversity no.2hospital from June2010to Dec2011. The90patients,according to the relevant disease diagnosis criteria strictly, divided into4groups: Viral encephalitis group40cases; Tuberculous Meningitis group15cases; Purulent meningitis group15cases; Normal control group20cases.All the cases were under lumbar puncture within48hours after admission (Viral encephalitis group require under lumbar puncture again afterhospitalization and medical treatment with7~10days). We sent cerebrospinalfluid to do routine, biochemical, cytology series examination and obtainedcerebrospinal fluid sample0.5ml in a-70℃refrigerator to determine the assayof NSE in cerebrospinal fluid under the NSE ELISA Kit which provided byUscn Life Science Inc, wuhan. Viral encephalitis group need to test clinicalmanifestation laboratory examinations such as blood routine, EEG, brain CT/MRI.Used SPSS13.0statistical package for statistical analysis of experimentaldata. Measurement data were measured by (x±s); All the groups' observedvalue were needed to take the normal test and the equal check of variance; Thecomparison of two samples mean with analysis of independent-samplet T Test,multiple mean with analysis of One-Way ANOVA; P <0.05show that thedifference between them being significant.Results:1The NSE level rang of Viral encephalitis group incerebrospinal fluid is10.63~20.02μg/L, average value is14.71±2.37μg/L;Normal control group is2.17~8.85μg/L, average value is6.26±2.34μg/L;Between the two groups, t=13.092, P=0.000<0.05, the difference aresignificance in statistics.2After treatment of Viral encephalitis group is7.18~14.68μg/L, averagevalue is12.18±2.24μg/L which decreased significantly compared with Viralencephalitis group; Between the two groups, t=3.701, P=0.001<0.05, thedifference is significance in statistics. But the content is higher than Normalcontrol group, t=-7.414, P=0.000<0.05, the two groups of the difference issignificance in statistics.3The NSE level rang of Tuberculous Meningitis group in cerebrospinal fluidis9.32~15.66μg/L, average value is12.80±1.70μg/L what decreasedcompared with Viral encephalitis group, t=2.858, P=0.006<0.05, thedifference are significance in statistics; But the content is higher than Normalcontrol group, t=-9.152, P=0.000<0.05, that the difference is significance instatistics. 4The NSE level rang of Purulent meningitis group in cerebrospinal fluid is10.42~16.59μg/L, average value is12.99±1.91μg/L what decreasedcompared with Viral encephalitis group,t=2.527,P=0.015<0.05,that thedifference is significance in statistics;But the content is higher than Normalcontrol group,t=-9.092,P=0.000<0.05,that the difference is significance instatistics.5Tuberculous Meningitis group compared with Purulent meningitis group,t=-0.284, P=0.778>0.05, that the difference is no significance in statistics.Conclusions:1The NSE level of Viral encephalitis patients incerebrospinal fluid were significantly greater than Normal control group, thedifference is significance in statistics; This show that human nerve cellsinfected with Virus will be degeneration and death, Thus results in irreversibledamage of nervous system function.2The NSE level of Viral encephalitis patients after treatment were lower thanbefore, the difference is significance in statistics; This show that DynamicObservations of the NSE level for condition monitoring and prognostic valueof viral encephalitis patients has important significance.3The NSE level of Viral encephalitis patients were higher than TuberculousMeningitis group and Purulent meningitis group, the differences aresignificance in statistics. This show that the lesion in the brain infected withvirus is more serious Compared with the others, so The content determinationof NSE plays an important value in differential diagnosis of Viral Encephalitis,Tuberculous Meningitis and Purulent Meningitis among which have manysimilarities in the clinical manifestation. But this experiment set Tuberculousmeningitis group and Purulent meningitis group are lesser in amount, we needincrease in case to advance study of brain damage between Tuberculousmeningitis group and Purulent meningitis group.
