Objective: To compare the efficacy and safety of extracorporeal shock wavelithotripsy (ESWL) and ureteroscopy lithotripsy(URL)for ureteral calculi;to find thebest treatment methods.Methods:75patients underwent ureteroscopy lithotripsy,including28sides ofcalculi1cm or greater and47sides of calculi less than1cm in diameter.75patientsunderwent ESWL, including33sides of calculi1cm or greater and42sides of calculiless than1cm. Respectively on two groups of cases of the therapeutic effect make therelated discussion and analysis.Results:The stone free rate after3months in patients with1cm or greater stonewas92.8%for ureteroscopy lithotripsy. It was higher than63.6%with ESWL (P <0.05). As for calculi less than1cm, stone clearance rates with ureteroscopy lithotripsy andESWL were95.7%and88.1%,respectively (P>0.05). In URL group, the upper andmiddle and lower ureteral stones clearance rates were63.0%,88.9%,96.7%, respectively,the3groups compare with the difference which was statistically significant.Conclusions:Our study demonstrates that ureteroscopy lithotripsy for calculi1cmor greater in diameter is more effective than ESWL,but no difference between the2groups for calculi less than1cm. In URL, the success rate of the treatment of middle andlower ureteral stones was significantly higher than that of upper ureteral. ESWL can beused as the first choice of treatment for upper ureteral calculi less than1cm in diameter.The middle or lower levels of ureteral calculi1cm or greater in diameter should bepreferred URL. But successful entrance of the ureterscope is a prerequisite and retrogradestone displacement is the primary reason for surgical failure.
