The reconstruction after the earthquake in May12,2008is a long andomplicated system engineering. The reconstruction and development of permanentesettlement community in recent years are the primary work of the reconstructionfter the earthquake. In the new permanent resettlement community, the lifeommunity of community residents had been disintegrated, the social relations are outf action, and the new social relations network has not yet formed. These unstableactors exacerbate the vulnerability of the disabled children in permanent resettlementommunity. The disabled in society is a large and special traditional vulnerable groups,isabled children and their families in the community are lack of discourse rights andon’t have the activities of resource extraction, so they will be on the verge of therisis which been called " social death". Therefore, the rehabilitation and growth ofhe disabled children are not only the hope of the disabled children’s family and theocial’s,but also are the important guarantee for maintaining the stability andarmony of the community and social.Based on long-term, disabled children’s rehabilitation is comprehensive, it shouldnclud medical rehabilitation, rehabilitation education and social rehabilitation.mong them, medical rehabilitation and education is the basis, and the socialehabilitation is the highest rehabilitation. The disabled children in permanentesettlement community are vulnerable, due to the living environment has beenhanged, and lacking the correct cognition, the disabled children will have obstaclesn self identity, community integration, social communication and social adaptation.n order to avoid the social death, the disabled children must realize the socialehabilitation through the proper channels.Community-based rehabilitation is a new rehabilitation, which referred as CBRas proposed by the world health organization in1976. Compared with the traditionalehabilitation in institutions, Community-based rehabilitation is not only suitable to local conditions, but also economic and easy. It will combine rehabilitation servicesand community support, and provide a barrier-free environment for the physicallydisabled children’s social rehabilitation in permanent resettlement community.Community-based rehabilitation from the perspective of social work, is not onlyprovide the rehabilitation services of social work, but also focusing on constructingthe community supporting networks, which to realize the social rehabilitation ofphysically disabled children. The author takes a physically disabled child in Zpermanent resettlement community for the case study, through interviews andnon-participatory observation methods collected data of physically disabled childrenand their family. Based on social work’s ethical principles called " self-determination"and"person-in-environment", the author adopted the " ecological map" and"important decision support network" method to define the physically disabledchildren’s problems. and formulated the corresponding rehabilitation programtogether. This article take "dominance" and" empowerment" theory as the theory ofsocial work’s intervention. The social work skills and methods will recover theself-confidence of the physically disabled children, construct normal peer network forthem, as well as empowerment physically disabled children’s family, promotingcommunity-based rehabilitation of them.In this paper, the logical sequence in general process model of social work isused as main thread to developing research framework. through a case study of socialwork practice, the specific process and methods of social work acrosscommunity-based rehabilitation of physically disabled children. Based on the research,this article will divide the practice study into five parts as establish relationships、assessment issues、develop service plans、counseling and treatment、outcome andterminate relationships. Establish relationships include agreement and assessmentissues, focusing on making trust relationship with physically disabled children andcommunity; assessment section includes the principles of assessment, assessmentmethod and defining main problem of physically disabled children; develop serviceplan part includes the rehabilitation program principles and plans; counselling andcorrection part focus on edisplaying the method of social work to intervent the community-based rehabilitation of physically disabled children; outcome andterminate relationships section includes assessment summary and the methods ofending professional relationships, mainly for the effect assessment and ending thecase, discussing how to terminate the work relations.The community-integration and community-participation of physically disabledchildren are important to the harmony and development of permanent resettlementcommunity. Social work across the physically disabled children’s rehabilitationcommunity, not only infusing vigor to community reconstruction work, but alsoconsciously promoting the localization professional development of social work,atthe same time, it widen the vision of disaster research, it will be a win-win process.
