ObjectivesThe study aims at assessing quality of life of civil servants in Guangdong, and evaluate there liability and validity of SF-36in application of QOL assessment among civil servants of Guangdong. To study the main factors which influence quality of life of civil servant in Guangdong.Methods:1SubjectsStratified sampling was carried on in2580civil servants in Guangdong during the second half of the year2010.2Methods(1) Operational definitionWorld Health Organization (WHO) considers Quality of Life to be "the individual’s perception of his or her position in life, within the cultural context and value system he or she lives in, and in relation to his or her goals, expectations, parameters, and social relations. It is a broad-ranging concept affected in a complex way by the person’s physical health, psychological state, level of independence, social relationships, and their relationship to salient features of their environment.’(2) InstrumentQuestionnaire of quality of life for civil servants was designed, which included SF-36and factors like demography, life way, and event. SF-36is an international popular scale about health-related quality of life, which is widely used by measurement about quality of life, evaluation of clinical trial effect, assessment of civil health policy and similar field. It was proved to be well in reliability and validity.(3) Quality controlThis research was supported by the Youth Foundation of Social Science Humanity of Education of China (NO:10YJCZH192) and the Planned Science and Technology Project of Guangdong Province in China (NO:2009B080701073).The sampling design and the statistical analysis were both performed according to the design of this subject to make sure the whole study was scientific and reasonable. Investigator explained to subjects how to fill in the scale before the test. Then subjects should asses themselves based on their understanding to each item independently without others’ influence. Investigators should not give suggesting guide. After the investigation, we selected the received questionnaires, rejected questionnaires which accomplishment ratio was under80%, writing quality was not high (for instance all of the questions were with the same answer) or two questionnaires shared totally the same answer. Database was established by Epidata3.02, and double input was conducted to make the lowest deviation.(4) Data processing and statistics①Data processingBuild up the database by Epidata3.02and process the data by SPSS15.0.②Evaluation methods of SF-36Reliability reflects the degree of variation caused by random error in the test. Common indicators of reliability include test-retest reliability, internal consistency reliability and split-half reliability. In this study, we evaluate internal consistency reliability and split-half reliability of SF-36, and the internal consistency reliability is evaluated by Cronbach’s a coefficient.Validity reflects accuracy of the test result, that is whether the scale can tell the subject’s health or not. Common indicators include construct validity, content validity, and criterion validity. In this study we evaluate construct validity of SF-36, which construct validity is evaluated by correlation of items and factor analysis.③Statistics Describe quality of life of Guangdong civil servants by Descriptive and Frequency module. To examine civil servants’ quality of life and its influencing factors t-tests, one-way analysis of variance (ANOVA) and multivariate stepwise regression analysis were performed.Result:1. The response rate is96.59%. Participants concerned content of the questionnaire, and they would like to finish it.2. Reliability and validity of SF-36(1) Internal consistency reliabilityCronbach’s a coefficient of SF-36is0.925. Cronbach’s a coefficient of physiological and psychological subscale were0.877,0.883respectively. Cronbach’s a coefficient of every dimension is not less than0.70out of’Social Functioning’(2) Split-half reliabilityThe spot trial results indicated a Spearman-Brown split-half reliability of0.821.(2) Construct validity①Correlation coefficient between each dimension and its affiliated items were bigger than that between the dimension and other items. Correlation coefficient between each subscale and its affiliated dimensions were higher than that between the dimension and other subscale.②Factor analysisKaiser-Meyer-Olkin index is0.939, suggesting that the data is suitable for factor analysis. According to the design of SF-36, extract9factors by principal axis factor analysis, whose cumulating contribution rate reach58.52%. After equamax rotation, result shows that every item has a high load value in one factor. The nine factors extracted match the theoretic conception, and every item of a dimension gathers together in the same factor.3. The average scores of SF-36of civil servants in GuangdongThe average scores of physiological and psychological of SF-36and its total score were (74.50±14.63),(70.85±16.15),(72.64±14.29) respectively. 4. The single factor analysis on influential factors of quality of life in civil servants of GuangdongThe difference of SF-36total scores was no statistical significance among different gender (t=1.275, P=0.202). The difference of SF-36total scores was statistical significance among different only age (F=9.340,P=0.000), marital status (F=5.662,P=0.004), affiliation property (F=12.958,P=0.000), job rank (F=3.624,P=0.006), work time (F=35.056,P=0.000), living place (F=5.906,P=0.003), smoking (t=20.671,P=0.000), drinking (F=845.811,P=0.000), eating habits (F=31.012, P=0.000), sleeping time (F=26.023, P=0.000), physical exercise (F=37.720,P=0.000), social intercourse (F=310.729,P=0.000), operating computer and sedentariness (F=20.477,P=0.000).5. Factors of quality of life in Guangdong civil servantsMultiple stepwise regressions showed the coefficient of determination of equation was0.579. That means the12variables from the multiple stepwise regressions can be combined to explain57.9.0%of total score. Inspection of the equation, F=211.058, P <0.001. The multiple stepwise regressions showed that factors, such as drinking, smoking, physical exercise, gender, work time, sleeping time, age, living situation, affiliation and Job rank related to civil servants’quality of life.(P<0.01).6. Relationship between health-related quality of life and sub-health state in GuangDong civil servants.There was a significant positive correlation between SHMS V1.0and physical, psychological level of quality of life. Canonical correlation analysis revealed that psychosocial symptom, vitality, positive emotion, physical mobility function, social adaptability and psychological were the main factors influenced SHMS V1.0and quality of life, respectively.7. Suggestions(1) Give a three-tier intervention plan on health management for Guangdong civil servants, according to the main factors affecting sub-health state of them.(2) Monitor, analysis, evaluate and forecast civil servants’health, so that to give relevant suggestions and system health management. (3) Civil servants and manage department should pay close attention to sub-health state of civil servants.Conclusions:1. SF-36shows good reliability and validity in test for Guangdong civil servants, it is suitable for quality of life evaluation among Guangdong civil servants.2. Civil servants of older, divorced or widowed, comprehensive political and administrative, bureau, working with longtime, live alone, smoker, drinker, having no breakfast, sleeping less, without physical exercise, always have working entertainment, operating computer and sedentariness have poor quality of life.3. Main factors that influenced civil servants’quality of life included drink, working entertainment, smoke, physical exercises, eating habit, operating computer and sedentariness.4. Sub-health state of GuangDong civil servants associated with quality of life. The more serious sub-health state was, the worse quality of life among GuangDong civil servants was.5. Give a three-tier intervention plan on health management for Guangdong civil servants, according to the main factors affecting sub-health state of them.
