Nowadays, chronic diseases has become an unavoidable public health problem. The influence of the chronic diseases on youths cannot be ignored. According to the special psychophysical characteristics of the youths suffered from chronic disease and the causes of chronic disease, the article discussed the intervention of social work so to provide feasible proposal on care and rehabilitation to the youths suffered from chronic disease. On the practice level, the intervention of social work can help medical workers and the youths suffered from chronic disease to deal with problems and tap the patients’potential so to help the patients to adapt society developing better.On the basis of interviews with the youths suffered from chronic disease, the patients’ family members and the doctors, the article summarized the special psychophysical characteristics of the youths suffered from chronic disease and analyzed the causes of chronic disease from heredity, environment, psychological, behavior and life style and health services. It is particularly necessary for social workers to provide intervention because the help from family, school, hospital and community are remaining inadequate. The article laid special stress on analyzing the social work intervention methods for youths suffered from chronic disease from cognition, behavior, mood and family. The article also analyzed the resources that can be used by social workers from family, school, enterprise, peer group, community and government and discussed the difficulties in intervention, which can let the social workers use the professional advantages to help the youths suffered from chronic disease and their families to face the challenge to reach help and self-help.
