Purpose Gastric cancer is one of the common malignant tumors in China,and gastriculcer is common disease of gastric mucosa lesions.People have been concernedabout the problem that distinguish between benign and malignant gastric ulcer andlooking for high specificity and sensitivity of tumor marker to detect early stomachcancer.To this end, the study by immunohistochemical detection of PTEN and survivinprotein expression in patients with no precancerous lesions of gastric ulcer group,precancerous lesions of gastric ulcer group and ulcer-gastric cancer group to furtherexplore its value in differential diagnosis of benign and malignant gastric ulcer and itssignificance in the evolution of ulcers.Methods Collect the second hospital of Hefei in January2006~June2011year clinicalinformation is complete and tissue samples preserved tissue samples a total of214casesof gastric ulcer.By using S-P immunohistochemical staining from different gastric ulcermucosa to test the expression of PTEN and survivin, including63cases ofnon-precancerous lesion,68cases of precancerous lesion and83cases of ulceratedgastric. Analyze its relationship to pathological characteristics of ulcerated gastriccancer, correlation between both, its value in differential diagnosis of benign andmalignant gastric ulcer and its significance in the evolution of ulcers.Results1.Expression of PTEN protein in benign gastric ulcer group, may also haveexpressed in malignant gastric ulcer.From gastric ulcer mucosa, of non-precancerouslesion to precancerous lesion to gastric cancer, the PTEN protein expression positiverate percent was98.41%,61.76%,and46.99%,respectively, shown a decliningtrend(P<0.01). 2.Expression of survivin protein in malignant gastric ulcer group, may also haveexpressed in benign gastric ulcer.From gastric ulcer mucosa, of non-precancerous lesionto precancerous lesion to gastric cancer, the PTEN protein expression positive ratepercent was25.40%,58.82%,and71.08%,respectively, shown a rising trend(P<0.01).3.The expression of PTEN was negatively correlated with the expression of survivin(r=-0.489, P<0.01).4.The expression of PTEN in patients with gastric carcinoma of ulcerative type norelated with the gender, age, ulcer site and Hp infection(P>0.05).The expression ofPTEN related with differentiation degree,higher differentiation than the poorlydifferentiated(2=6.103,P<0.05).The expression of PTEN related with the serumCEA level,higher normal group than the elevated CEA group(2=4.275,P<0.05).5.The expression of survivin in patients with gastric carcinoma of ulcerative type norelated with the gender, age, ulcer site and Hp infection (P>0.05).The expression ofsurvivin was lower in differentiation than the poorly differentiated, but difference notstatistically significant (P>0.05).The expression of survivin was lower in normal groupthan the elevated CEA group,but difference not statistically significant (P>0.05).6.In detection of ulcerated gastric carcinoma, the specificity of PTEN and survivin was79.39%,57.25%, the sensitivity was52.38%,71.08%, respectively.7.Survivin protein expression related with pathological evolvement (P<0.05).PTENprotein expression not related with pathological evolvement (P<0.05).Conclusion From gastric ulcer mucosa, of non-precancerous lesion to precancerouslesion to gastric cancer, the expression of PTEN tended to decreasement (P<0.01),survivin expression tended to increasement.The expression of PTEN was negativelycorrelated with the expression of surviving (r=-0.489, P<0.01). The expression of PTENrelated with differentiation degree and the serum CEA level(P<0.05), and survivinprotein expression related with pathological evolvement (P<0.05).
