Expression And Significance Of Survivin And PTEN In Ovarian Epithelial Neoplasms

Objective: Oophoroma is one of the three types of malignant tumors of organa genitalia feminine and it causes the highest rate of death in malignant tumors of gynecology, the 5-year survival of patients in all stages of the disease is only 30%～40% . Oophoroma is symptomless at clinical early stage, so it is hard to detect its histological category and whether it is malignant or not. Accordingly the study of development of oophoroma is important for its diagnosis, treatment and prognosis. survivin gene is an apoptosis inhibiting gene. It can inhibit apoptosis and accommodate caryocinesia. The recently study indicated that the survivin protein played an important role in the development of oophoroma and the expression of survivin protein was related to its clinical stage, histological grading and lymphatic metastasis. PTEN(phosphatase and tensin homolog deleted on chromosome ten), the Chinese people on chromosome 10 called the missing phosphatase and tensin homologue gene ,it is the first found inhibiting gene that has dual specificity phosphatase activity. It can inhibit phosphorylation of tyrosine and make a negatively regulation to cell multiplication. Quantity studies have confirmed that PTEN gene was related to tumor`s emberyonic phase, angiopoiesis, local infiltration, metastasisi, degree of differentiation and the size. At the present, the accuracy relationship between survivin, PTEN gene and epithelium oophoroma is not clear.This study is to detect the expression of survivin and PTEN in epithelium oophoroma by immunohistochemical method. According to reports, both in gastric cancer ,esophageal cancer, colorectal cancer, endometrial cancer, cervical cancer, brain cancer and other malignant astrocytoma pathogenesis , there are also a role and relationship.To detect the expression of survivin and PTEN in epithelium oophoroma. Analyze the relationship between the two and each clinical pathological indes of epithelium oophoroma. Invest the effect and the relation of the two between the development of oophoroma and the significance of the two in diagnosis and treatment of epithelium oophoroma.Methods:Immunohistochemical analysis SABC was performed to detect the expression of survivin and PTEN in 24 cases of epithelium oophoroma,10 cases of epithelium borderline ovarian tumors,10 cases of ovarian innocent tumor and 8 cases normal ovarium. Analyze the consequence and invest its clinical significance.Result:1. 70.83% epithelium oophoroma showed survivin positive reaction. It is manifestly higher than the positive reaction rate in epithelium borderline ovarian tumors(40.00%) or ovarian innocentumor(10.00%) or normal ovarium(0%).And the difference had statistically significant(p<0.05).2. The relationship between the expression of surviving and tumor`s clinical stage,histological grading,pathological category and lymphatic metastasis: 50.00% epithelium oophoroma at stageⅠ—Ⅱshowed survivin positive reaction while 85.71% at stageⅢ—Ⅳ.And the difference had statistically significant(p<0.05).53.33% epithelium oophoroma in G1—G2 showed survivin positive reaction while 100% in G3.And the difference had statistically significant(p<0.05).75.00% mucous oophoroma and 70.00% serosity oophoroma showed survivin positive reaction and 66.67% other oophoroma(clear cell carcinoma,endometrioid carcinoma)showed survivin positive reaction.And the difference had no statistically significant(p>0.05).40.00% oophoroma without lymphatic metastasis and 92.86% oophoroma with lymphatic metastasis showed survivin positive reaction.The difference had statistically significant(p<0.05).3. 33.33% epithelium oophoroma showed PTEN positive reaction.It is manifest lower than epithelium borderline ovarian tumors(50.00%), ovarian innocentumor(70.00%)and normal ovarium(100%).And the differences had statistically significant(p<0.05).4. The relationship between the expression of PTEN and tumor's clinical stage,histological grading,pathological category and lymphatic metastasis: 70.00% epithelium oophoroma at stageⅠ-Ⅱshowed PTEN positive reaction while 7.14% at stageⅢ-Ⅳ.And the difference had statistically significant(p<0.05).46.67% epithelium oophoroma in G1-G2 showed PTEN positive reaction while 11.11% in G3.And the difference had statistically significant(p<0.05).25.00% mucous oophoroma and 40.00% serosity oophoroma showed survivin positive reaction and 33.33% other oophoroma(clear cell carcinoma,endometrioid carcinoma)showed PTEN positive reaction.And the difference had no statistically significant(p>0.05).60.00% oophoroma without lymphatic metastasis and 14.29% oophoroma with lymphatic metastasis showed survivin positive reaction.The difference had statistically significant(p<0.05).5. There is relevance between the expression of survivin and PTEN in epithelium oophoroma.It is positive correlation(χ2=4.27 , p<0.05 ,λ=0.87).Conclusion:1.Survivin expression and PTEN expression downgraded and ovary epidermis tumor occurrence correlation, and has the synergism in the ovary epidermis tumor occurrence development.2.Survivin in ovary epidermis tumor expression and clinical by stages, histology graduation, lymph node shift present correlation.3.PTEN in ovary epidermis tumor expression and clinical by stages, the histology graduation, the lymph node shift assumes the inverse correlation.4.Survivin and PTEN assume the inverse correlation in the ovary malignant epidermis tumor expression, examines the survivin gene and the PTEN gene jointly, regarding the appraisal ovary epidermis tumor malignant degree, the shift and the prognosis has the clinical value, becomes the ovary epidermis tumor hopefully the effective diagnosis target and the treatment new target spot.
