Objective:Diabetic nephropathy (DN) is the most common microvascular complications indiabetic patients. In the early stage of DN, there is no obvious clinica symptoms andabnormal changes of routine urinalysis and markers of renal function appear, butkidneys may have minor injuries.Therefore early detection of renal damage in diabeticpatients is significant. To investigate the clinical value of sodium dodecyl sulfate-agarose gel electrophoresis(SDS-AGE) in early diagnosis of diabetes nephropathy, byanalysizing markers of urine and serum in diabetic patients.Methods:According to the1999World Health Organization (WHO) Diabetes MellitusDiagnostic Criteria, there are fifteen healthy adults in control group, onehundred-thirty-two patients in diabetes mellitus group. According to urine proteinqualitative analysis,132cases of diabetes are divided into five groups, Pr (–)65cases,Pr (﹢﹣)32cases, Pr (1+)16cases, Pr (2+)10cases, Pr (3+)9cases. We usedsodium dodecyl sulfate-agarose gel electrophoresis (SDS-AGE) to detect theunconcentrated urine on all of patients with diabetic mellitus urine and healthy adultsin control group, and by using laboratory analytical analysize urinary NAG,24hurinary albumin and serum biochemical indexes including BUN, CRE, Cys-C andRBP.Results:1、Urine protein electrophoresis results of healthy adults in control group2cases have pale-staining albumin bands, namely physiologic proteinuria; the other13patients without any protein bands.2、Urine protein electrophoresis results of patients with diabetic mellitus thereare4cases appeared mixed proteinuria,1cases appeared glomerular proteinuria,16cases appeared tubular proteinuria,29cases appeared physiologic proteinuria,15 cases did not appear any protein band in65cases Pr (–) patients;32cases of Pr (﹢﹣)patients:9patients with mixed proteinuria,7patients with glomerular proteinuria,6cases tubular proteinuria,10cases with physiological proteinuria;16Pr (1+) appearedin7patients with mixed proteinuria and Glomerular proteinuria occurred in3cases,1case of tubular proteinuria,5cases of physiological proteinuria; there are5casesappeared mixed proteinuria,3cases appeared Glomerular proteinuria,1caseappeared tubular proteinuria,1case is physiological proteinuria in10cases Pr (2+)patients;9patients of Pr (3+):4cases of mixed proteinuria,4cases of glomerularproteinuria,1case of tubular proteinuria.3、Biochemical indexes in diabetic patients65cases of Pr (-): the positive caseof serum BUN is11, the positive case of SCr is0, the positive case of Cys-C is7, thepositive case of RBP is0, the positive case of urinary NAG is32, the positive case of24h urine protein is15;32cases of Pr (+-): the positive case of serum BUN is5,the positive case of SCr is1, the positive case of Cys-C is4, the positive case of RBPis1, the positive case of urinary NAG is21, the positive case of24h urine protein is12;16cases of Pr (1+): the positive case of serum BUN is7, the positive case of SCris1, the positive case of Cys-C is3, the positive case of RBP is1, the positive rate ofurinary NAG is14, the positive case of24h urine protein is10;10cases of Pr (2+):the positive case of serum BUN is5, the positive case of SCr is1, the positive case ofCys-C is3, the positive case of RBP is1, the positive case of urinary NAG is7, thepositive case of24h urine protein is9;9cases of Pr (3+): the positive case of serumBUN is5, the positive case of SCr is4, the positive case of Cys-C is5, the positivecase of RBP is1, the positive case of urinary NAG is7, the positive case of24h urineprotein is9.Conclusion:1、Non-concentrated proteinuria electrophoresis of SDS-AGE is a sensitive, rapid,non-invasive, direct-viewing detection method.2、Non-concentrated proteinuria electrophoresis of SDS-AGE for the earlydiagnosis of diabetic nephropathy, treatment, prognosis is positive, practical clinical value.3、Combined detection of variety of biochemical markers in diagnosis of diabetickidney damage is more valuable.4、some sensitive indicators of renal damage in the practical application has notget satisfactory result, so we need to do a detailed and comprehensiveassessment.
