Objective: In recent years, with the rapid development of social andscientific technology, resulting in open injury patients were increasing, and nowpeople often use3%hydrogen peroxide solution (H2O2) in the clinical surgicaldebridement. Its role in the sterilization of surgical debridement has long beenrecognized, but it was also caused by injury to the body to bring it into question.In this study, we producted animal models of open injury accompanied byperipheral nerve injury, and used hydrogen peroxide solution to treat fracturedsciatic nerve tissue of the rabbits. Observed content variation of myelin basicprotein(MBP) in the nerve tissue, electrophysiological functional recovery andmicroscopic pathologic change with hematoxylin-eosin staining. Understandingwhether hydrogen peroxide solution increase injury to fractured sciatic nervetissue that can guide the application of hydrogen peroxide solution in the clinicalsurgical debridement in order to reduce the hydrogen peroxide solution furtherdamage to peripheral nerve injury, shorten postoperative cycle of neurologicalfunction recovery, and ultimately reduce patient postoperative pain. Methods: Select24healthy adult rabbits (male or female, aged1.5-2yearsold, weight2.5-3.0kg, purchased from the Experimental Animal Center ofGuilin Medical College), randomly divide them into A, B, C, D four groups(each group has6). Cut off the sciatic nerve of the right side and thenanastomose it. After cutting off the sciatic nerve, use hydrogen peroxide solutionto rinse the sciatic nerve of Group A and C for1minute, then use physiologicalsaline rinse it, Group B and D just rinse with physiological saline as a control. InGroup A and B, take the segment of injured nerve tissue and measure content ofMBP in it after2days. In Group C and D, get the excised sciatic nerve tissueand measure electrophysiological conduction velocity of it after12weeks. Alsotake a piece of normal sciatic nerve tissue, cut it into to two segments averagely.One segment treated with hydrogen peroxide solution for1minute and thenrinse with physiological saline. And the other segment just rinse withphysiological saline as a control. Two of them were doing routine nerve tissuebiopsy and HE (hematoxylin and eosin) staining, in contrast to make an opticalmicroscope to observe the changes in the nerve tissue.Results: The content of MBP of the sciatic nerve tissue which treated withhydrogen peroxide solution was significantly higher than that without thetreatment of hydrogen peroxide solution(P<0.05). After12weeks of recoverytime, the electrophysiological conduction velocity of the sciatic nerve tissuewhich treated with hydrogen peroxide solution was significantly lower than thatwithout the treatment of hydrogen peroxide solution(P<0.05). In themicroscope image of the pathological sciatic nerve tissue, compared with thesciatic nerve tissue without the treatment of hydrogen peroxide solution, the nerve sheath membrane was obviously destroyed, and also the nerve fibermyelin inside the nerve sheath membrane was dehydration shrinkage.Conclusions:(1) Hydrogen peroxide solution can promote MBP levels risein the injured sciatic nerve tissue. This may be related to the H2O2promotingWallerian degeneration to proximal and distal nerve fibers of the fractured nervetissue.(2) Application of hydrogen peroxide solution will preventelectrophysiological function recovery of injured sciatic nerve. This may berelated to the damaging effect of H2O2on cell membrane structure, what candamage to the structural basis of the electrophysiological function of nervefibers. It also may be due to the involvement of H2O2obstructing the axonaltransport of nerve fiber between neuronal cell body and axon, thus preventingthe repair of damaged nerve fibers.(3) Hydrogen peroxide solution has obvious injury to sciatic nerve tissue: itcan directly damage the sheath structure of nerve tissue and also bring somedamage to the myelin sheath of nerve fiber.
