Objectives To observe the impact of change of High sensitivity c-reaction protein(hs-CRP) on carotid artery plaque in middle-aged and elderly people.Methods A total of5852individuals were randomly stratified from the101510healthexamination survey participants in Tangshan Kailuan Company community during2006~2007.A total of5440people(age above40years old,free of stroke,TIA andmyocardial infarction were enrolled in the final analysis.A questionnaire survey,bloodbiochemical analysis and carotid artery ultrasound examination were finished by trainedmedical staff.Finally,a total of4656participants were analyzed.According to thehsCRP measurements in the two times health examinations between2006to2007and2010to2011,we divided observation participants into four groups,including continueddecreasing group, decreasing group,increasing group and continued high group.Multiplelogistic regression analysis was used to test the impact of change of hs-CRP on carotidartery plaque.Results The detection rate of plaque of continuous low group, reduced group, elevatedgroup and high group of hs-CRP were33.6%、44.7%、45.1%and54.9%.In addition tohs-CRP low group and elevated group had no significant difference in the outer, the restof two groups have significant differences.(P<0.05).Single factor logstic regressionanalysis showed that the hs-CRP packet for dummy variables, with hs-CRP for low groupas the reference group. In the overall population, hs-CRP group, hs-CRP decreased, hs-CRP increased group sustained high group, the carotid artery plaques were moredangerous40%、42%and111%(RR=1.40,95%CI1.14-1.73;RR=1.42,95%CI1.16-1.75;RR=2.11,95%CI1.65-2.70);In the female population, hs-CRPdecreased, hs-CRP increased group, group hs-CRP sustained high group, the carotidartery plaques were more dangerous with84%、66%and165%(RR=1.84,95%CI,1.35-2.52;RR=1.66,95%CI,1.17-2.36;RR=2.65,95%CI,1.81-3.88);Inthe male population, continued high hs-CRP group compared with the continuous lowgroup increased risk of carotid artery plaques (RR=1.97,95%CI1.41-2.76).Adjusted for age, sex, LDL-C, FBG, smoking, drinking, physical exercise, systolic bloodpressure, triglyceride, BMI and other factors in multivariate logstic regression analysis showed, in the overall population, and hs-CRP continued low group, hs-CRP low group,elevated group were increased carotid artery plaques dangerous40%'36%(RR=1.40,95%CI1.09-1.80; RR=1.36,95%CI1.06-1.73); In the femalepopulation in the lower hs-CRP group elevated group, the carotid artery plaquesdangerous low group were increased compared with hs-CRP62%'69%(RR=1.62,95%CI1.11-2.35;RR=1.69,95%CI1.13-2.54);In the male group, and hs-CRP forlow group, hs-CRP low group, elevated group sustained high group, the carotid arteryplaques were not increased risk.Conclusions The concentration of hs-CRP increased or decreased may be risk factorsfor carotid artery plaque.
