Objective To analyse the characteristics of choroid melanoma progression. To explorethe diagnostic value of color doppler flow imaging for choroidal melanoma, pathologicalbasis of choroidal melanoma color flow signals, relationship between color flow signalsand tumor malignancy and the main prognostic factors.Methods During the period from January2005to March2014in Tianjinophthalmology hospital,31cases choroid melanoma patients were retrospectivelyanalyzed.31patients all underwent preoperative color doppler ultrasonography, andpostoperative pathological section for confirmation. Color doppler ultrasound measurethe items including tumor itself’s blood supply characteristics, distribution of blood flowand hemodynamic parameters (PSV.EDV.TAPV.RI.PI). CDFI is used to display thenumber of blood vessels, the size and shape of the tumor. The items above werecompared with that in pathological section results. The results obtained through twomethods were compared and analyzed to explore their correlation.Results1With the increasing days of the course, vision was gradually losing. Whenthe naked visual acuity≥0.05, the duration of course is1~8months and16patients areall at intraocular stage, sclera foreign invasionⅠperiod; naked visual acuity<0.05, theduration of course is2~40months, about60%(9/15) of the choroid melanoma patientshad entered sclera infringement Ⅱ period, Ⅲ period and Ⅳ period, consistent withthe clinical stage.2The number of blood flow signals of CDFI compared with that inpathological section, the difference was statistically significant (P<0.05). According tocolor flow grading standards the consistency of them is90.3%.3Comparison on the sizeof tumor through color doppler ultrasound and pathology had no obvious statisticalsignificance (P>0.05) with consistency of87.1%.4Nodules choroidal melanomas inextraocular spread stage are all large humors accompanied by a large number of cellnecrosis, which mostly have not rich blood.5Follow-up and prognosis:2005to2011there were surgical patients of19cases in total, follow-up of12cases,11cases survived(2cases with eye skin melanosis), and1case died28thmonth postoperatively with livermetastasis. From2012to2014, there were12cases patients in total,11cases survived1case death as diffuse choroid melanoma with liver metastasis for17months. Conclusions1The onset of choroid melanoma hides.87.1%of patients see a doctorbecause of vision loss. The degree of vision loss and staging of scleral invasion arepositively associated with the duration of course.2CDFI can auxiliary diagnose choroidmelanoma. the internal blood flow grade of tumor reflected by CDFI are highlyconsistent with vessel conditions in pathological section.3CDFI can reflect actual size ofhumor. The similarity rate of humor shape reflected by CDFI and humor shape inpathological section is87.1%.4Nodules choroidal melanomas in extraocular spreadstage are all large humors accompanied by a large number of cell necrosis, which mostlyhave not rich blood, have higher trend of orbital spread and have higher risk of systemicmetastasis.5Blood flow characteristics of choroid melanoma are different from otherparts of the tumor. The peripheral portion is greater than the central portion. uppertemporal part is slightly more than the others. The humor with no rich blood flow mayhave poor prognosis.
