Association Of Carotid Atherosclerosis And Coronary Artery Disease

Objective To Study the relationship of carotid atherosclerosis and coronary atherosclerosis and evaluate the value of carotid ultrasound in predicting high-risk population of coronary artery disease(CAD) and people who needed to be examined further by coronary angiography.Methods We enrolled 165 cases suspected of CAD hospitaled in The first people’s hospital of Chenzhou from August in 2013 to October in 2014. Carotid ultrasonography and coronary angiography were completed in those 165 patients. Compared and analyzed the results of carotid ultrasonography and coronary angiography, detected the relationship between the atherosclerosis of carotid artery and coronary artery, evaluated the ability of carotid ultrasound in prediction of CAD.Result When diagnosed a patient had CAD with the criteria of carotid ultrasonography showed carotid atherosclerosis with/without carotid atheromatous plaque, the sensitivity and specificity were 87.9% and 61.0%, The total correct rate was 81.2%, P<0.001. In order to evaluate the extend of the stenosis of coronary artery, when diagnosed a patient had serious CAD(the stenosis of coronary >75%) with the criteria of carotid ultrasonography showed carotid atherosclerosis with/without carotid atheromatous plaque, the sensitivity and specificity were 90.6% and 40.0%, The total correct rate was 66.1%, P<0.001, otherwise when diagnosed a patient had serious CAD(the stenosis of coronary >75%) with the criteria of carotid ultrasonography showed carotid atherosclerosis with carotid atheromatous plaque, the sensitivity and specificity were 64.7% and 71.3%, The total correct rate was 67.9%, P<0.001.Conclusion Carotid atherosclerosis has good correlation with the atherosclerosis of coronary artery. The high-risk population of CAD can predicted by carotid ultrasonography, when carotid ultrasonography showed carotid atherosclerosis, it’s suggested starting secondary prevention of coronary heart disease, when carotid ultrasonography showed carotid atherosclerosis with carotid atheromatous plaque, it’s suggested having a coronary angiography to know exactly the extend of the stenosis of coronary and weather or not need coronary artery revascularization.
