Objective : The pathogenesis of osteoporosis is not yet clear. The occurrence and development of osteoporosis relate to bone metabolism related hormones, nutrition(including must nutrients and trace elements), physical and genetic, environmental and other factors. Currently, there is few epidemiological research of osteoporosis related risk factors. This study try to make the relationship of bone mineral density and osteoporosis risk factor of osteoporosis clear through the investigation of the general situation, history of disease, family history and the way of living in patients,and detecting blood biochemical index, bone mineral density, bone markers and the levels of serum trace elements of osteoporosis patients in 309 hospitals. Thus,this investigation can provide more guidance to the clinical diagnosis and treatment of osteoporosis.Methods: Selecting hospital or outpatient clinical patients during December 2013 to December 2013 in the 309 as research objects. General condition, the history of disease, family history and way of living of patients with osteoporosis were obtained through the questionnaire survey, blood biochemical index and bone turnover markers and serum levels of trace elements were detecting in venous blood, the bone mineral density using of lumbar spine, femoral neck and total hip were detecting by dual-energy X-ray absorptiometry. Combining the result of questionnaire survey,blood biochemical index, bone transformation indexes, serum levels of trace elements and bone mineral density make the relationship of bone mineral density and osteoporosis risk factor of osteoporosis clear with two categories of Logistic regression and correlation analysis and Pearson correlation.Results:1. the total of questionnaires is 430 cases, recycling effective questionnaire is 392 cases,and the effective rate is 91.2%. The observation group is 289 cases,and the control group(not diagnosed with osteoporosis) is 103 cases. The youngest among all the respondents is 47 years old, the oldest is 85 years old; Man is 109 cases,accounting for 27.8% of the total, woman is 283 cases, accounting for 72.2% of the total.2. Logistic regression analysis to questionnaire content, including height shorter,using hormones and menopause, family history of early menopause, brittle fracture,smoking, drinking alcohol are the risk factors of osteoporosis, OR values is 1.970,1.096, 1.661, 1.023, 1.146, 1.299, 1.146, and milk and sunlight are the protection factors of osteoporosis, OR values is 0.935, 0.935, 0.935, and the others has no correlation with the incidence of osteoporosis in this study.3. Waist circumference is negatively correlated with Troch bone density trend,suggesting centrality obesity is bad for Troch bone growth.4. Pearson correlation analysis between bone mineral density and bone metabolism index, according to the results of BAP and lumbar spine and hip BMD( including L2,L3 and L4, Neck, Troch, Ward’s) are positively correlated trends with statistical significance(p < 0.05), the rest of the bone metabolism indexes(including OC and CTX) are positively correlated with lumbar spine and hip BMD without statistical significance(p > 0.05).5. Bone mineral density and blood biochemical indicators between the Pearson correlation analysis results show that:(1) the ALT, AST and lumbar spine and hip BMD(including L2, L3 and L4, Neck, Troch, Ward’s) are positively correlated;(2)the CRE, GFR and L2, L3 and L4, Neck, Troch trend of bone mineral density are positively correlated, BUN and L2, L4 trend of bone mineral density are positively correlated;(3) Ca, P, K and the lumbar spine and hip BMD(including L2, L3 and L4,Neck, Troch, Ward’s) are positively correlated trends, Mg, Na, Cl and L2, L3 and L4,Troch, Ward’s trend of bone mineral density are positively correlated;(4) UA, TC, TG,HDL and lumbar spine and hip BMD(including L2, L3 and L4, Neck, Troch, Ward’s)are positively correlated trends, LDL and L2, L3 and L4, Troch, Ward’s trend of bone mineral density are positively correlated, GLU and lumbar spine and hip BMD(including L2, L3 and L4, Neck, Troch, Ward’s) negatively correlated trends,apolipoprotein A1 and negatively correlated with L2, L3 bone density trend of apolipoprotein B and L2, L3 and L4, Neck bone mineral density showed a negative correlation trend, with statistical significance(p < 0.05), the other has no correlation with bone density.6. Pearson correlation analysis between bone mineral density and serum trace elements, according to the results of V, Cr, Mn, Fe, Cu, Zn, Se and lumbar spine and hip BMD(including L2, L3 and L4, Neck, Troch, Ward’s) were positively correlated trends, Pb and lumbar spine and hip BMD(including L2, L3 and L4, Neck, Troch,Ward’s) were negatively correlated trends, Ni and L2, L3, Troch bone mineral density was positively correlated trends, As and L2, Neck, Troch bone mineral density was positively related trends with statistical significance(p < 0.05).Conclusions:1. shorter height, using of hormones, menopause, family history of early menopause,brittle fracture, smoking, drinking are risk factors for osteoporosis, and milk and sunlight are protection factors for osteoporosis, other factors remaining no correlation with the incidence of osteoporosis.2. Waist circumference and negatively correlated with Troch bone density trend,suggesting centrality obesity is bad for Troch bone growth.3. BAP and bone mineral density are positively correlated, and it is the representative index of bone formation, more sensitive to bone formation compared to otherindicators, which we can through the detection of BAP understand bone metabolic state.4. A variety of biochemical indexes and correlated with bone mineral density: ALT,AST, CRE, GFR, BUN, Ca, P, K, Mg, Na, Cl, UA, TC, TG, HDL, LDL and bone mineral density are positively correlated, GLU, apolipoprotein A1, apolipoprotein B negative correlation with bone density, while the present study results show that the correction of abnormal liver and kidney function, electrolyte and blood lipids index is easy to disease of heart head blood-vessel and systemic stability of internal environment, but excessive correction may cause adverse effects on bone.5. V, Cr, Mn, Fe, Cu and zinc, Se and many parts bone mineral density are positively correlated, trace element supplement for tips to try through the prevention and treatment of osteoporosis; Pb are negatively correlated with all parts bone density, tips to reduce the intake of Pb to prevent osteoporosis.
