Objective To explore the expressions and clinicopathological characteristics of OPN and related factors:CD44v6, interstitial CD 10, CD68 and stromal collagen expression in breast cancer tissues, and analyze their respective correlation with prognostic values.Methods A retrospective analysis of clinical pathological data on 28 cases of breast adenosis (as control),40 cases of DCIS and 153 patients with invasive carcinoma of NST. Immunohistochemical of EnVision two-step staining was used to examine the differential expressions of OPN, CD44v6 and interstitial CD 10 and CD68 in the breast tissues mentioned above, and the stromal collagen proliferation was detected with masson staining, and analyzed their relationships with clinicopathologic factors and prognostic values. Kaplan-Meier method was used to estimate patients’ prognosis (DFS and OS), and the Log-rank test was adopted to compare the intergroup differences of measurement data.Results(1) In breast adenosis, DCIS and NST, the positive rates of OPN were 7.1%,27.5% and 56.2% respectively, while of CD44v6 the positive rates were 10.7%,40.0% and 57.5%, and the positive rates of CD10 were 3.5%,37.5% and 55.6%, and the number of tumor-associated macrophage(TAM) (CD68 positive) in these three breast tissues were 19.7±8.8/HPE 65.1±7.6/HPF、84.9±24.5/HPF respectively, and the result of Masson staining showed that expression rate of collagen in different breast tissues were (39.8±18.2)%、(62.0±19.3)%、(66.5±15.7)%.(2) The expressions of OPN and its related markers in breast adenosis were lower than those in breast cancer, showing significant differences (P<0.05). In NST tissues, the positive rates of OPN, CD44v6 and CD 10 were all correlated with histological grade, lymph node metastasis and pTNM stages, and CD44v6 was related to PR status (P<0.05); OPN, CD44v6 and CD 10 expression were all related to histological grade in DCIS (P<0.05); while thses three markers had no relation with patients’age, menopausal status, vessel invasion, Ki-67 proliferation index, ER and HER-2 status (P>0.05), both in DCIS and NST patients, and CD68 positive TAM was only related to pTNM stage and histological grade in NST (P<0.001, P=0.006), while the results of Masson staining indicated that expression rate of collagen was only associated with tumor size in DCIS (P=0.012). In breast cancer tissues, OPN, CD44v6 and CD 10 had positive association with each other (P<0.001).(3) In NST cases, survival analysis was carried out on OPN, CD44v6 and CD 10 respectively, OS were better in CD44v6 negative group than that in positive group, and DFS, OS were better in CD 10 negative group than those in CD 10 positive group, and the differences were of statistically significance (P=0.043,0.006,0.004); DFS and OS were better in the triple negative group (OPN, CD44v6 and CD 10 all negative) than those in triple positive group (P=0.010,0.007). Multivariate analysis showed that lymph node metastasis and CD 10 expression were prognostic factors of NST patients.Conclusion Invasion and high metastasis risks of breast cancer are characterized by the high expression of OPN and its related factors. Survival analysis shows that a joint detection of OPN, CD44v6 and CD10 expressions is of great value in predicting patients prognosis. Therefore, further studies on OPN and related factors may provide a new method for breast cancer therapy.
