Research Of Case Management Applied In The Urban Disabled Elderly People

Disability is an inevitable social phenomenon in the historical process of human society. The disabled group is a large and vulnerable group, the "weak" and "marginalized" level is still high. Therefore, the community has been widespread concern in society. But there is a group in society which combines two special characteristics, has not received enough attention, they are disabled elderly population. Because group of disabled elderly people belong to the disabled and the elderly, the demand has a dual character and diversity. Therefore, the servicers have to provide integrated services to meet the needs of the complex group. Case management is to provide social work model of integrated services for individuals or families with multiple needs and problems. Through a combination of social workers, community, civil affairs departments, medical institutions and social groups and resources, as the case to provide targeted, comprehensive service.This study selected two cases of the management mode of professional social work services for the disabled elderly cases, analysis the specific case of the implementation of the management, to explore the application effect of the model in the disabled elderly social work services and advantages. Case management implementation process used the evaluation, coordination, supervision, resource links and other ways to help solve practical problems, and improve the weak state.Finally, the study found that through the implementation of case management, that can help the client to solve real-life problems, helping to establish and strengthen the social support network, enhance the quality of life. Compared with the general case, case management can be more effective to provide integrated services, is conducive to the realization of social work and the existing docking community work. At the same time, the study also found that the smooth implementation of case management needs to be built on the basis of community service system is relatively perfect, and the existing social service system is not perfect is hindered the main reason case management implementation and development. Therefore, the development and promotion of the new method of case management still faces many obstacles and challenges.
