Clinical Analysis And Treatment Strategy Of Endovascular Aortic Repair In Treatment Of Complex Abdominal Aortic Aneurysm

Objective : To investigate the technical essentials and efficacy of endovascular aortic repair(EVAR)in treatment of complex abdominal aortic aneurysm(AAA).Methods:Selected and analyzed the clinical data of 27 patients with AAA,who had been underwent endovascular repair in the First Affiliated Hospital of Medical University Of Anhui between January 2011 and June 2016.Divide the patients into two groups:complex group and ordinary group.Compare the preoperative general data,preoperative treatment and fellow up result of the patients of the two groups,and analyze the difference between two groups.Results: There were 13 patients in complex group and 14 patients in ordinary group.The average age of the patients in complex group was older than ordinarygroup(72.69±8.30)years old>(70.21±8.54)years old.The percentage of pain symptoms,the average diameter and length of aneurysm and the surgery time,average hospitalization expenses in complex group was higher than ordinary group(6>1 ?(9.10±2.10)cm>(7.31±1.60)cm ?(125±59)min>(100±61)min ?186310±33276>162080±22412).The difference between two groups have statistical significance(p < 0.05).The surgical successful of two group was 100%.The number of patients who had endoleak in complex group and ordinary group were 5 cases and 2cases during preoperative period.The follow up of two group range from 1 month to 12 months,mean follow-up rate was 76.9% and 71.4%,average follow-up time was(8.9±0.8)and(8.8±0.8)months.During the follow-up period,there were 2 cases with endoleak in complex group and 1 cases in ordinary group,none of the patients died.It showed that have no statistical significance between two groups in above aspects.Conclusions : EVAR is a feasible method for the treatment of patients with high-risk and complex AAA.The average number of aneurysm diameter,length,operation time and the hospitalization costs of the two groups had statistical significance.
