Study On The Symptom Dimensions And Psychological Defense Mechanism Of OCD

Object: To study the symptom dimensions and psychological defense mechanism in patients with obsessive-compulsive disorder,and to explore whether there is association between them,in order to provide the suggestions for clinical treatment.Methods: 76 patients met the diagnostic criteria of ICD-10 diagnosis of obsessive-compulsive disorder,the Yale Brown Obsessive Compulsive Scale of more than 16 points were selected,and complete the general information,Yale Brown obsessive compulsive symptom checklist(Y-BOCS-SC)and defense mechanism questionnaire(DSQ)assessment,compare the clinical data of patients with different symptom categories,symptom dimensions,and the data were analyzed statistical.Results:(1)The score of immature factors of OCD group is significantly higher than normal control group(P<0.05),and the score of mature defense mechanism is significantly lower than normal control group(P < 0.05).(2)Factor analysis showed that the 4 major dimensions(variance contribution rate)are as follows: symmetry / ordering(19.609%),contamination / washing(17.602%),hoarding(14.112%),harming / checking(13.590%),the cumulative variance contribution rate is 64.912%.(3).The single factor analysis of variance(ANOVA)was used to analyze the dimensions of OCD symptoms and psychologicaldefense mechanism.The results showed that there is no significant difference between the dimensions of symptoms and psychological defense mechanism(P >0.05).Conclusions:(1)When people in face of conflict or frustration,OCD patients are more use immature defense mechanisms(such as projection,acting out,complaining,fantasy,splitting)and idealization of intermediate defense mechanism compared with normal people;less use mature defense mechanisms(such as sublimation,depression,humor).It is necessary to carry out psychological intervention on patients with obsessive-compulsive disorder,so as to change the coping strategies and improve their prognosis.(2)Obsessive-compulsive disorder have heterogeneity,through factor analysis to get the 4 symptom dimensions,namely: symmetry / ordering,contamination / washing,hoarding,harming / checking.The clinical manifestation of patients with different symptoms are different,which may provide some classification evidence for the clinical and research of the subtypes of OCD.(3)There was no relevancy between the dimensions of OCD symptoms and the types of psychological defense mechanism.
