Objective: To evaluate the clinical value of SUDOSCAN in diagnosis of diabetic peripheral neuropathy by comparing with the clinical diagnostic criteria of diabetes peripheral neuropathy.Methods : According to the diagnostic criteria for multiple diabetic symmetrical neuropathy,130 patients with type 2 diabetes were divided into two groups,diabetic peripheral neuropathy group(DPN group,50 cases)and not diabetic peripheral neuropathy group(NDPN group,80 cases).All these 130 patients were from the department of Endocrinology,the Affiliated Hospital of Qingdao University.The healthy control group included 80 healthy volunteers from the department of physical examination center,the Affiliated Hospital of Qingdao University(NC group,80 cases).Sudoscan was employed to detect electrochemical skin conductance(ESC,the unit was ?S)of subjects' hands and feet,and the differece would be compared using single factor analysis of variance comparison between each group.The correlation between sensory nerve conduction velocity(SCV)and ESC was analyzed,and the correlation between the duration of diabetes and ESC was analyzed as well.The correlation between age and the duration of diabetes was analyzed at the same time.Results:(1)All participants in the process of SUDOSCAN test did not fell any uncomfortable feeling,and the palms of hands and feet did not become red.There were no adverse events happen in the participants in the inspection.(2)The average hands ESC in NC and type 2 diabetes mellitus group were(79.1±10.4)?S and(59.7±18.1)?S,respectively;the average feet ESC were(82.0±8.2)?S and(62.1±21.8)?S,respectively;(both P<0.01).(3)The average hands ESC in DPN and NDPN were(53.2±18.9)?S and(63.7±16.5)?S;the average feet ESC were(53.5±24.4)?S and(67.4±18.2)?S,respectively,(both P<0.01).(4)The hands and feet ESC were positively correlated with SCV,and the correlation coefficient were 0.425 and 0.445,respectively(both P<0.01).(5)The hands and feet ESC were negatively correlated with the duration of diabetes,and the correlation coefficient were-0.350 and-0.428(both P<0.01).(6)The duration of diabetes was positively correlated with the age of diabetes,the correlation coefficient was 0.330(P<0.01).(7)The area under the receiver operating characteristic curve(ROC)of hands ESC to evaluate diabetes peripheral neuropathy were 0.785,the cutoff value was 64.5?S,the sensitivity and specificity was 72.0 % and 73.7%,respectively.The area under the receiver operating characteristic curve(ROC)of feet ESC was 0.768,the cutoff value was 60.5?S,the sensitivity and specificity was 60.0% and 83.7%,respectively(both P<0.01).Conclusions:(1)The test results of SUDOSCAN are closely related to the duration of diabetes,and the secrete sweat nerve damage will be heavier with the longer of the duration of diabetes,the hands or feet ESC value will be lower.(2)SUDOSCAN detection metho amination).(3)SUDOSCAN is a promising tool for the diagnosis of diabetic peripheral neuropathy of Chinese.
