The Clinical Characteristics Of Inverted Papilloma Of Bladder And The Significance Of Irrigation In The Later Treatment

Objectives: 1.By analyzing the bladder through the clinical data of patients with inverted papilloma,summarizes the general clinical characteristics and incidence of the disease.2.Comparated observation of transurethral resection of bladder tumor combined with epirubicin bladder perfusion and pure in transurethral resection of bladder tumor in the bladder papilloma of curative effects,explore the optimized treatment plan,reduce the recurrence rate of malignant transformation,reduce the incidence of late symptoms.Methods: 1.Collected Qinghai University Affiliated Hospital and Qinghai Provincial Peoples Hospital and The first peoples hospital in Xining and Qinghai Hospital of Traditional Chinese Medicine from March 2011 to November 2016 pathological biopsy results clearly diagnosed as suffering from the inverted papilloma in bladder,all cases were collected for general situation High risk Clinical manifestations of clinical characteristics of the research on the several aspects such as the analysis summary,reveals the occurrence characteristics.2.Put the cases into two groups,regard the transurethral bladder tumor resection combined with bladder perfusion of epirubicin as the observation group(group A),regard the simply transurethral resection of bladder tumor as the control group(group B),regular fall off line urine cytology after surgery Urinary pathological biopsy or cystoscopy related inspection,record the results.Compare two groups of patients with recurrence and progression,recurrence and malignant transformation rate calculation;late record the occurrence of symptoms,compared the incidence of discomfort.Results: 1.Collected a total of 64 patients,as follows: Male 53 cases,11 cases of women;23 cases less than 50 years old,41 cases were greater than or equal to 50 years old;Clinical manifestations of intermittent painless gross hematuria in 31 cases,Clinical characterized by frequent urination urgency urinary pain in 17 cases,Clinical manifestations of dysuria in 6 cases,Clinical manifestations of lower abdominal discomfort of lower back by subhealth in 2 cases,no obvious clinical symptoms in 8 cases;Routine urine test results: white blood cells + ~ + + + + a total of 23 cases,red blood cell +-~ + + + + a total of 41 cases;multiple bladder tumor 1 case,can be seen in the bladder lesions of three size,size of 10 mm×13 mm,14 mm×21 mm,25 mm ×36 mm.single bladder tumor in 63 cases,5 cases in the left side of the bladder,8 cases in the right side of the bladder,6 cases at the bottom of the bladder,34 cases located at the bladder neck,11 cases located in the bladder trigone.The tumor has 21 cases within 10 mm diameter,tumor diameter in 39 cases of 10 mm to 30 mm,tumor diameter greater than 30 mm in 4 cases;appearance of the tumor is base on wide nipples or polypoid in 26 cases,appearance of the tumor is a finer coral samples or algae in 38 cases.2.Group A have a total of 34 cases,2 cases of recurrent,1 case of malignant transformation,the rate of recurrence is 5.88 percent,Malignant transformation rate is 2.94 percent,Group B have a total of 30 cases,3 cases of recurrent,2 case of malignant transformation,the rate of recurrence is 10.00 percent,Malignant transformation rate is 6.67 percent.Two groups of recurrence rate and malignant transformation rate comparison,the difference had no statistical significance.Conclusions: 1.IPB often occurred in the male,women rarely,the sex ratio of 6:1,often onset age level in middle age.2.IPB onset reason not clear,with long-term repeated bladder inflammatory stimulation all sorts of reasons lead to lower urinary tract obstruction of HPV(HPV)infection chemical carcinogen factors and is closely related factors such as smoking.3.IPB with painless gross hematuria as the most,followed by frequent urination urgency urinary pain dysuria or lower abdominal discomfort symptoms,and presence of clinical symptoms of patients obviously.4.IPB common good hair position is located in the trigone?neck and bilateral ureteral orifice area,the prostatic urethra are relatively rare,top bladder is a rare.5.The diagnostic methods of IPB were cystoscopic examination and histopathological biopsy.6.IPB often single,occasionally happens,tumor diameter within 3 cm more.7.IPB is relatively good prognosis,recurrence and malignant transformation rate is low.8.In the aspects of recurrence rate and malignant transformation rate,but the difference had no statistical significance.
