Clinical Research On Prevention Of Postoperative Recurrence Of Superficial Bladder Cancer By Bladder Perfusion With Combilation Hemorrhoid Injection And Hydroxycamptothecin

Objective:1.To furtherly explore the effictive dose and mechanism of using hemorrhoid injection with hydroxycamptothecin to prevent superficial bladder tumor:probably this combination can cut down the patients' blood VEGF level.2.To observe the clinical curative effect and safety of the combination of hemorrhoid injection and hydroxycamptothecin of different dose to treat superficial bladder tumor.Methods:1.The groups of case:According to the selected standard,we select 90 patients whom been after operation and divide 3 groups at random,every group have 30 patients.Group A:patients received a single dose of 20mg hydroxycamptothecin;Group B:patients received 10ml hemorrhoid injection and 10 mg hydroxycamptothecin; Group C:patients received 10ml hemorrhoid injection and 20mg hydroxycamptothecin;2.Empirical method:After operation,the instillation wille repeated one time one week for two months and one time one month for ten months.3.The index:To observe the index before operation and 3 months,6 months and 12 months after operation.A:BUS,cystoscopy,Chest X-ray review(CT or MRI p.r.n);B:Scores of KPS;C:The blood VEGF level;D:Blood RT,Urine RT,Liver and renal function,Biochemistry;E:Record the adverse effect;Results:Before and after reperfusion in A,B,C three sets of data for comparative study,the results are as follows:1.Relapse rate:A group 23.3%(7 / 30),B group 13.3%(4 / 30),C group and 13.3%(4 / 30).B,C groups,respectively,compared with the A group,there are significant difference(P＜0.05).Note that the joint applications of hemorrhoid injection and Hydroxycamptothecin,can reduce tumor recurrence;change the dose of HCPT,the relapse rate without significant change.2.VEGF levels:A,B,C three groups before and after reperfusion, no significant difference(P＞0.05)Perfusion method of three groups that can not reduce the level of serum VEGF. 3.KPS score comparison:A group with the B,C groups,there are significant difference(p＜0.05),B group and C group no significant difference(P＞0.05)Note B,C two groups of patients in improving quality of life is better than A group;B group and C group in the good quality of life in patients with no significant difference.4.Adverse reactions:A,B,C groups compared three groups no significant difference(p＞0.05).Perfusion methods that each group of adverse reactions in no significant difference.Conclusions:The joint applications of hemorrhoid injection and Hydroxycamptothecin in bladder chemotherapy to reduce tumor recurrence and improve the quality of life of patients and did not increase the incidence of adverse reactions.The mechanism may be little relationship with VEGF.
