Given the context of several social challenges,such as population aging,old aging,sickness and disabilities,China has to focus on the reformation of institutional pension system.The diverse needs of the elderly,such as medical services,life care,hospice care and other increasing needs of the elderly,cannot be satisfied with family pension.There are a variety of attempts all over the country,which contribute to a trend of integration of medical care and pension services.Serval factors was found in the current administrative system which constraint the integration of medical care and services.Through literature research of the classical theory of bureaucracy,the public policy implementation theory and the new public service theory,agreement between the current medical support integration organization system and the operation mechanism of security system.According to the value judgment made with these relevant theory,proposed restricting China's integration of medical care and pension services was constraint by the organization system,the operation system and the security system.The development of the integration of medical care and pension services was jointly constraint by the internal factor of the organizational system and the external factors of the operation system and the guarantee system.The constraint was caused by organization division,overlapping functions and the disunity of the organization interests in the organization system.More concretely,civil affairs department and health department has problems such as cross in management,uniform of policy implement and insufficient participation of hospital for the reason of different belonging of pension services and the health cares.And the lack of health assessment mechanism,decisionmaking mechanism and human capital cultivation contribute to the constraint of the operation system,including the sequence of the integration has not formed,diversity of the main responsibility,human resources shortage and so on;In the security system,lack of funds,supervising system and medical insurance system also constraint the development of the integration of pension services and medical care,the main reason is insufficient financial investment of the government,lack of medical insurance system and legal regulation.These constraint was enriched with actual cases,this paper put forward the key approaches to improve the organization system,the operation system and the security system.The improvement of the organization system mainly through the unified organization of the organization,the establishment of the elderly Work Bureau,so as to divide the clear responsibilities,through the policy to motivate the hospital to participate in with preferential policy.The operation system need to structure the pension services and standard the decision maker.The security system need to improve supervisory system by law,set up long-term insurance and encourage social capital.
