Schizophrenia is a common mental illness but with the highest prevalence, often with the disturbance of perception, thoughts, feelings behavior and other aspects of the disorder and uncoordinated mental activity. It has a prolonged course and the prognosis is poor. In China, a large number of patients are unable to access to hospitalization, and many hospital patients whose condition is stable but because of the lack of personalized rehabilitation, strand in hospitals leading to a vicious cycle of mental illness treatment conditions.This paper takes Nanjing Q psychiatric hospital as an example and through case management probes into the rehabilitation process of mental illness that the professional social work intervene in. The number of hospitalized persons in Q hospital has reached saturation point, the embarrassing situation occurs "community mental patients can not stay in the hospital patients can not be discharged." This situation not only go against patient’s re-socialization, adding family, hospitals and government’s economic burden, but also deny the right of psychiatric patients admitted to hospital for treatment and receiving medical care.The study objects of this article are chosen as a schizo who has reached the discharge standards, a mental hospital patient guardian and Q. By using in-depth interviews, observation, and literature method to understand their living conditions in hospital, he hospital will, CBR difficulties, the paper develops rehabilitation plans for the patients with schizophrenia to improve their life skills taking the advantage perspective theory and hierarchy of needs theory as the theoretical basis; through some ways to increase patients" social and vocational skills and then improve their self-confidence. which may promote their return to normal life and in turn to address economic pressures of mental hospital; the use of community work methods can improve the social inclusiveness for rehabilitee of mental illness, promoting their social integration, and increase the patient’s discharge rate to meet the rest of the community hospitalized patients needs.I, as Q psychiatric social workers, co-work with the hospital specialists, nurses, and community workers to develop an rehabilitation program for client and help improve his self-medication management skills, work skills and life skills. After a period of rehabilitation, the client’s work and life skills has significantly been improved, and he now can take the medicine on time.
