Doctor-patient trust matters a lot to social stability and distinguishes between public and private hospitals.Qualitative methods were used to explore the differences in organizational structure and daily operation between two kinds of hospitals, and how those differences influence doctor-patient communication modes, both of them induce doctor-patient trust into two different traits. On the one hand, public hospital is classified as institutional organization for its emphasis on the integration of institutional elements,so patients' initial trust is kind of systematic trust based on institution. However, those institutional elements cannot completely promote public hospital's performance, which causes the decoupling of organizational structure and its daily operation.Furthermore, the lack of team solidarity and the competition among doctors make patients' trust turn from systematic to interpersonal level. On the other hand, based on the integration of basic institutional elements, private hospital pays more attention to construct network of relationships to enhance organizational performance,so to some extend it can be classified as relational organization, and patients' initial trust is kind of interpersonal trust. In contrast to the decoupling of public hospitals, private hospital's structure and daily operation show a coupled state,in which doctors of one medical team cooperate well with each other and have no conflict of interest.Besides, this private hospital is more concerned with emotional care for patients than the public one.Gradually, patients' trust for this private one turn from interpersonal to systematic level during or after the treatment.Systematic trust is wide while interpersonal trust is deep, the complement of each other can enhance patients' trust, but more endeavor need to be made.In a long run,the decoupling state of public hospital will deteriorate patients' systematic trust, which urges the organization to optimize institutional elements and improve structure's rationality with government's supports. As for the private hospital, patients' systematic trust is still at an emergent stage which makes it necessary to continuously improve its medical level besides those service departments and to construct a higher institutional status in order to widen patients' systematic trust.
