At present, our country has basically established the basic medical insurance system of universal coverage, some slow improvements are emerging in the situation in difficulty and high cost of getting medical service. However, with the increasing demand of clinic and rapidly rising medical costs, the pressure of medical insurance fund pay is aggravated, the safety of the medical insurance fund is under threat. In order to ensure smooth growth of fund operation, all parts began to actively explore various forms of the reform of the medical insurance payment.First, this article picks Tangshan as the object of study. As a national and provincial level demonstration city, the experiences of global budget reform have generalizability.This article is based on the understanding of Tangshan medical insurance development and Global Budget running situation, through survey and interview to get the first-hand data. The analysis centers the changes in before and after reform of hospitalization expenses, the number of services, service quality, hospital management behavior, patient visit, the change of medical insurance fund utilization, etc, in order to analysis the problems. The causes of the problems are in six factors: hospitals' profit-seeking,hospitals' imperfect management measures, health care informatization level is not high,not setting up health-care negotiation platform, publicity of medical knowledge is not enough, health supervision mechanism is not sound. At the end, the article puts forward some improving advanced suggestions. Through the effective experience of the implementation of Global Budget and problem analysis, the writer offer some experiences for other regions for reference.
