OBJECTIVE:To study the effect and mechanism in different anesthetic methods on wound healing in canine.The clinical effect and the mechanism in different anesthetic methods on cutaneous wound healing in canine were discussed from the perspective of wound,cell and the expression of mRNA and protein of cytokine.Methods:Fifteen dogs were randomly divided into three groups:intravenous injection of Shumianning and maintenance anesthesia(group S),intravenous injection of propofol with isoflurane inhalation anesthesia(group I),intravenous injection of Shumianning with local anesthesia of 2%lidocaine(group L).In the experiment,experimental cutaneous trauma model is caused on canines’ back,routine debridement in postoperative wound debridement.Additional drug therapy is not used in wound.On the 1st,3rd,5th,7th,10th,15th and 20th days,the dogs were anesthetized in the same way and the wound tissue was collected for pathological examination at each time point.In order to explore the effect of three anesthetic methods on the wound healing factors,the wound tissue of canines was sampled at different time points.The mRNA expression level of the correlative cytokines FGF-2,TGF-β1 and VEGF-A was detected using the method of real-time fluorescence PCR.The protein expression of FGF-2,TGF-β1 and VEGF-A was detected by immunohistochemistry.Results:The results of each test showed that the overall observation of group L was the best in wound healing effect,the fastest healing of skin tissue.Newborn granulation tissue appeared the earliest,wound contraction was rapid,with anti-infective ability.The healing of group S appeared to be slow in the early stage of trauma and the healing rate was lower than other anesthesia groups.The apparent effect of group I was better than that of group S.Wound volume healing rate L>I>S.The number of each group had increased neutrophil process,each peak in the 5-7d.Group L had the fastest regression of inflammatory cell infiltration,strong anti-inflammatory ability,inflammation of the transition to the fast growth phase.The number of fibroblasts of each group showed a wave-like upward trend.The fibroblast cells of group S in 3d was more than others,but purulent effusion was showed in some wounds and the healing rate did not rise.The expression of TGF-β1 mRNA in group I was high at each time point,and the protein expression was strongly positive,indicating that inhalation anesthesia promoted the secretion of TGF-β1 and promoted the differentiation of fibroblasts and myofibroblasts,wound contraction and scar formation,endothelial cell division and matrix deposition.The mRNA expression of three cytokines in group L at all time points were high,strongly protein expressing,promoting the role of various cytokines,promoting capillary production,enhancing microcirculation,accelerating the inflammatory response,rapidly accelerating the proliferative phase,promoting granulation tissue formation,promoting wound contraction and epithelialization.The mRNA and protein expression of cytokine in group S was lower in early time,and gradually increased.In 10d,the expression of VEGF in group S was high,promoting the formation of angiogenesis and microcirculation enhancement.But this promotion occured later than that in group I and L.Conclusion:The ability of Shumianning intravenous injection anesthesia in canine cutaneous wound healing is weak,wound healing slowly.Intravenous propofol with isoflurane inhalation anesthesia can promote canine cutaneous wound healing,but the healing effect is weak than that of intravenous injection of Shumianning with local infiltration anesthesia of 2%lidocaine.Intravenous injection of Shumianning with local infiltration anesthesia of 2%lidocaine can effectively promote the healing of canine cutaneous wound.It has value in clinic popularizing.
