Objective: To refine the theoretical research and the clinical optimum treatment regimen of combined therapy of Chinese and Western medicine on diabetic peripheral neuropathy by organizing and studying the literatures including the Chinese and Western medicine pathogenic mechanisms,animal experiments and clinical treatment(Chinese medicine treatment,Western medicine treatment,combined therapy of Chinese and Western medicine)of diabetic peripheral neuropathy,thereby provides more effective diagnosis and treatment thinking and methods for clinical treatment of diabetic peripheral neuropathy.Methods: The literatures in recent ten years(2009-2018)related with the Chinese medicine theories,Western medicine theories,combined theories of Chinese and Western medicine,animal experiments and clinical treatment of diabetic peripheral neuropathy from databases were selected,classified,and researched to find out the theoretical research and the clinical optimum treatment regimen of combined therapy of Chinese and Western medicine on diabetic peripheral neuropathy.Results: The pathogenic mechanism of diabetic peripheral neuropathy was not yet clarified at presen,which predominantly related to metabolic disorders caused by hyperglycemia,nerve ischemia and hypoxia,deficiency of neurotrophic factor,immune factors,etc.,the Western medicine treatment methods mainly included improving metabolic disorders,improving microcirculation,resisting oxidation,nourishing nerves,stopping pain according to the symptoms,and surgical procedures.The disease was belong to Xiaokebi disease in the field of Traditional Chinese medicine due to Qi and blond yin-yang deficiency and stagnation of phlegm and blood,the Chinese medicine treatment methods mainly included decoction,Chinese patent medicine,Chinese medicine injection,acupuncture and moxibustion,acupoint application,acupoint injection,ultrasound therapy,iontophoresis,fumigation,and massage,etc..The literatures related to the experimental researches on cell apoptosis,cell signaling transduction,genomics,proteomics,metabonomics showed that,the occurrence of diabetic peripheral neuropathy was the result of multiple factors and multiple pathological reaction,its cause and pathogenic mechanism were very complex,and further researches were needed.But the literatures related to the combined therapy of Chinese and Western medicine on diabetic peripheral neuropathy indicated that,the clinical efficacy of combined therapy of Chinese and Western medicine was better than that of single Chinese medicine treatment or single Western medicine treatment.In this research,we proposed the theoretical research and the clinical optimum treatment regimen of combined therapy of Chinese and Western medicine on diabetic peripheral neuropathy by organizing and studying the literatures,which includes Chinese medicine treatment of benefiting qi and yin for activating blood circulation,eliminating phlegm and freeing channels,and combined with Western medicine treatment of reducing blood glucose and symptomatic treatment.Conclusion: The combined therapy of Chinese and Western medicine on diabetic peripheral neuropathy has complementary advantages compared with single single Chinese medicine treatment or single Western medicine treatment,which improves clinical efficacy;and we refine the theoretical research and the clinical optimum treatment regimen of combined therapy of Chinese and Western medicine on diabetic peripheral neuropathy,including Chinese medicine treatment of benefiting qi and yin for activating blood circulation,eliminating phlegm and freeing channels,and combining with Western medicine treatment of reducing blood glucose and symptomatic treatment,which is worth promotion and application.
