[Objective]The external nasal morphology of the patients with nasal morphologic changes caused by cartilage graft deformation after rhinoplasty using autologous costal cartilage was measured,and the characteristics and laws of external nasal deformation were analyzed and summarized.[Methods]1.From January 2015 to March 2018,we retrospectively analyzed the cases of rhinoplasty using autologous costal cartilage in our center.According to the criteria of inclusion and exclusion,31 cases were included,including 28 females and 3 males,aged from 19 to 43 years old,with an average age of 28.4 years old.Surgical reasons:14 cases of congenital flat nose,17 cases were because of dissatisfactory appearance or anxiety about material safety after rhinoplasty with other materials.2.By combining optimized plane photography and image-pro plus 6.0 software and according to the soft tissue measurement items of head and face defined by Farkas,11 soft tissue markers were selected,4 pairs;10 linear distances,1 pair;8 ratio indexes,1 pair;13 angle measurement items,5 pairs.Because of the same effect of left and right side measurement items,measured values of the right side is adopted.3.According to the age,the beauty seekers were divided into two groups:Group A1(19-30 years old)and Group A2(21-43 years old),Group A1 includes 21 beauty seekers while Group A2 includes 10;according to the surgery,the beauty seekers were also divided into two groups:Group S1(primary rhinoplasty)and Group S2(second rhinoplasty),Group S1 includes 14 beauty seekers while Group S2 includes 17;according to the side,left and right sides.Using SPSS 24.0 software,t test was used to compare the difference of measured values among groups,and the characteristics and laws of external nasal deformation were analyzed and summarized.[Results]1.Compared with the data of Group A1 and Group A2,there was no significant difference in the ratio indexes,angle measurement and symmetry indexes between the two groups(p>0.05).2.Compared with the data of Group S1 and Group S2,the difference between nasal tip protrusion rate and nasal-facial angle was statistically significant(p<0.05),that is,the nasal tip protrusion rate and nasal-facial angle in the secondary rhinoplasty group were higher than those in the primary rhinoplasty group.however,there was no statistical difference in other measurement items(p>0.05).3.The differences between angle 5 and angle 7 were statistically significant,indicating the deviation of nasal tip,mainly right.The dorsal deviation was mainly "C" type deviation which occurred in 18 cases,the mean values were 170.01 ±3.25°,"I" type deviation in 12 cases,"S"type deviation in 1 case.The mean values of nasal column deviation angle is approximately 3.00±2.03°mainly right deviation.[Conclusions]Rhinoplasty with autologous costal cartilage augmentation can effectively improve the external nasal morphology,making the proportion of nasal subunits and the relationship between nose and other facial organs more harmonious.There was no significant difference in angle measurement items,ratio indexes and symmetry indexes between Group A1 and the Group A2,and the age was positively correlated with the degree of calcification of costal cartilage.There is no change in degree of deformation due to age or the degree of calcification of costal cartilage.Compared with primary rhinoplasty,those who had a history of rhinoplasty with other materials may have more obvious nasal tip protrusion and larger nasal facial angle,indicating more obvious nasal depth.So in the rhinoplasty with autologous costal cartilage,we can adjust the height of nasal tip properly under the premise of satisfying the individual aesthetic appeal,pay more attention to prevent the "C" type deviation of nasal dorsum,right deviation of nasal tip and nasal column.
