Objective: To explore the effect of the prescription of the old-fashioned Chinese medicine doctor Zhou Yuzhu on the wound healing after acne mammary abscess and its possible mechanism through clinical research, and provide a theoretical basis for extensive clinical application. Methods: Sixty patients admitted to the Department of Breast Surgery of Anhui Provincial Hospital of Traditional Chinese Medicine from November to February 2018,2017 were diagnosed with Acne mammary abscess(with or without local ulceration)and 59 patients who met the inclusion crite rivanol.According to the random number table method, the patients with acne mammary abscess were divided into two groups: 30 patients in the treatment group(treated with Digouqufu liquid)and 29 patients in the control group(treated with rivanol).Treatment: After the surgical contraindications are excluded, the breast abscess or the breast abscessectomy or segmental gland resection is performed according to the specific range of the patient’s lesion. On the second day after surgery, the treatment group was replaced with a smear of sputum in the wound cavity, and the control group was replaced with a rivet yarn.During the treatment, the VAS pain score, wound secretion score, wound healing rate, number of fibroblasts and new blood vessels in granulation tissue, clinical efficacy and safety index were observed at each time point after treatment.Statistical analysis was performed using statistical software(SPSS 21.0).Result: 1.Before the treatment, the general conditions of the two groups of patients, namely the age and the original area of the wound, were compared. The difference was not statistically significant(P>0.05),which was comparable.2.Results after treatment:(1)Postoperative wound VAS pain score in patients with Acne mammary abscess: compared with rivanol group on the third day after treatment,t=1.0,P=0.32>0.05,compared with rivanol on day 21,t=0.2,P=0.84>0.05,no significant difference;on day 7 compared with the rivanol group,t=2.36,P=0.02,compared with the rivanol group on the 14 th day,t=2.05,P=0.04,significant difference.(2)Comparison of postoperative wound secretion scores in patients with Acne mammary abscess: compared with rivanol group on the third day after treatment,t=1.71,P=0.09>0.05,no significant difference;Day 7 and rivanol group In comparison,t=3.49,P=0.001<0.05,compared with the rivanol group on the 14 th day,t=2.90,P=0.005<0.05,compared with the rivanol group on the 21 st day,t=2.75,P=0.009<0.05,There are significant differences.(3)Comparison of postoperative wound healing rate in patients with Acne mammary abscess: On day 7 compared with rivanol group,t=3.02,P=0.04<0.05,on day 14 compared with rivanol group t=2.37,P=0.023<0.05,On day 21,compared with the rivanol group,t=2.14,P=0.038<0.05,there was a significant difference.(4)The number of fibroblasts in wound tissue was observed under light microscope: compared with the rivanol group on the 7th day after treatment,t=2.445,P=0.018<0.05,compared with the rivanol group on the 14 th day,t=2.258,P =0.029<0.05,there is a significant difference.(5)Observing the number of new blood vessels in the wound tissue under light microscope: Compared with the rivanol group on the 7th day after treatment,t=2.53,P=0.018<0.05,compared with the rivanol group on the 14 th day,t=2.22,P= 0.029<0.05,there is a significant difference.(6)Total postoperative evaluation of Acne mammary abscess patients: After 28 days of treatment, the overall effective rate of the Digouqufu liquid group was 96.7%,of which 6 cases(20%)were cured and 15 cases(50%)were effective.8 cases(26.7%),1 case(3%)were ineffective, and the effective rate was 86.2% in the rivanol group,13 cases(44.8%)were effective,12 cases(41.4%)were effective, and 4 cases(13.8%)were ineffective. The rank sum test showed that Z=2.602 and P=0.009,which were significantly different.(7)There were no uncomfortable reactions in the two groups during the total treatment. Conclusion: The clinical efficacy of external application of Digouqufu liquid in promoting wound healing in patients with Acne mammary abscess is better than that in rivanol group. The mechanism of promoting wound healing may increase the number of fibroblasts and the number of new blood vessels in wound tissue.related.The antifouling and antiseptic solution is simple, convenient, economical, and safe for postoperative wound dressing. It can be widely used in clinical practice.
