Study On The Objective Efficacy Evaluation And Mechanism Of Qigui Huoxue Decoction In The Treatment Of Type 2 Diabetic Peripheral Neuropathy

Objective :(1)To observe the clinical objective effect of qigui huoxue decoction in the treatment of diabetic peripheral neuropathy(DPN)with Qi deficiency and blood stasis;(2)Through the effect of qigui huoxue decoction on levels of SOD,MDA,IBIL and et-1 in patients with DPN of Qi deficiency and blood stasis type,the mechanism of the effect of Qigui Huoxue Decoction on DPN was explored;(3)Through this clinical study,it provides theoretical basis and data support for Chinese medicine treatment of DPN.Methods: Sixty patients with DPN of qi deficiency and blood stasis type who were included in the exclusion criteria of this study were enrolled in the control group and the treatment group according to the random number method.30 patients in each group,the control group and the treatment group were given diabetes diet,exercise and individualized hypoglycemic treatment.Control group: mecobalamin 0.5 mg / time,3 times / day orally,treatment group: control group based on the combination of Qigui Huoxue Decoction(200ml,in-hospital decoction,2 times a day),continuous observation of three courses(12 weeks).Observe the changes of biochemical indicators,curative effect indicators and safety indicators of blood sugar and blood lipids before and after treatment,and analyze and statistically describe the data.Results:(1)After treatment,the levels of FPG,2hPG and HbA1 c in the two groups were lower than those before treatment(P<0.01).The decrease of FPG and 2hPG in the treatment group was greater than that in the control group(P<0.01).There was no difference in HbA1 c between the two groups(P>0.05).(2)After treatment,TG,TC and LDL-C in the treatment group decreased compared with before treatment(P<0.05),and the levels of HDL-C in the treatment group was no difference than before treatment(P>0.05);the TG and TC of the control group were lower than those before treatment(P<0.05).The LDL-C and HDL-C of the control group were not significantly different from those before treatment(P>0.05).The reduction of TG,TC and LDL-C in the treatment group was larger than that in the control group(P<0.05),but there was no significant difference between HDL-C and the control group(P>0.05).(3)After treatment,the serum MDA and ET-1 levels in the two groups were lower than before treatment(P<0.05),and serum SOD and IBIL levels were higher than before treatment(P<0.05).The decrease of MDA,ET-1 and the increase of SOD and IBIL in the treatment group were higher than those in the control group(P<0.01).(4)After treatment,the MNDS score and TCM syndrome grading score of patients in the two groups were decreased(P<0.01).In the treatment group,the decline of MNDS and TCM syndrome grading score were greater than that of the control group(P<0.01).(5)After treatment,the MNCV and SNCV of the median nerve and common peroneal nerve were increased in the two groups(P<0.01).The increase of median nerve MNCV,peroneal nerve MNCV and median nerve SNCV in the treatment group were greater than that in the control group(P<0.01),while there was no significant difference in SNCV of common peroneal nerve between the two groups(P>0.05).(6)The total effective rate was 86.7% in the treatment group,which was significantly higher than 70.0% in the control group(P<0.05).(7)Safety analysis: There was no significant difference in the incidence of adverse reactions between the two groups(P>0.05).Conclusion: Qigui Huoxue Decoction has remarkable curative effect on DPN of Qi deficiency and blood stasis type.It can regulate blood sugar and lipid in DPN patients,improve nerve conduction velocity and clinical efficacy,improve oxidative stress level and endothelial function,improve clinical symptoms and sign,and has high safty.
