ADC Values In Identification Of Hyperplasia And Adenocarcinoma And Its Correlation With Adennocarcinoma Gleason Score

Objective:To explore MRI ADC values of identification of hyperplasia and adenocarcinoma in prostate peripheral zone and the correlation with adenocarcinoma Gleason score.Method:Collection in October 2014 to February 2016 the ninth people's hospital affiliated to Shanghai jiaotong university school of medicine?north?clinical suspicion of prostate cancer patients,as by transrectal ultrasound guided biopsy results as the standard,diagnosis of 41 cases of adenocarcinoma,hyperplasia of 43cases.All of the patients in the centesis on MRI.The hyperplasia of adenocarcinoma and 2 groups,the difference of ADC values between ROC curve drawing,explore the best diagnosis of adenocarcinoma of the ADC values for threshold,the evaluation of the ADC values in diagnosis of adenocarcinoma of the school?specificity and sensitivity?.According to the Gleason scores of prostate cancer and malignant nodules can be divided into low level?high differentiation?group?Gleason score<7?,medium level?medium differentiation?group?Gleason score=7?and the level of high?low differentiation?group?Gleason score>7?,to compare different differentiated adenocarcinoma 3 groups,the difference of ADC values between Spearman correlation analysis was used to evaluate the correlation of ADC values and Gleason score.Results:1,Hyperplasia of comparing adenocarcinoma and ADC values differences between the two groups have statistical significance?t=9.228,P=0.000?,and draw the ROC curve,measured area under the ROC curve is 0.869,Youden index calculation,when Youden index's biggest ADC values threshold of 1.39 x 10^-3 mm²/s,diagnostic sensitivity and specificity of adenocarcinoma was 71.43%and 89.33%.2,Comparison,adenocarcinoma of low level?high differentiation?group,medium level?medium differentiation?group and high level?low differentiation?ADC values differences between 3 groups with statistical significance?F=35.990,P=0.000?,and comparing three groups of patients,differences between groups were statistically significant?P₁=0.000,P₂=0.000,P₃=0.002?.3,ADC value of adenocarcinoma is negatively related with Gleason score?r=-0.701,P=0.000?.Conclusions:1,ADC values of adenocarcinoma and hyperplasia of the diagnostic threshold of 1.39 x 10^-3 mm²/s,specificity and sensitivity of 89.33%and 71.43%.2,It is valuable that ADC values assess the malignant degree of carcinoma.3,ADC values are negatively related with Gleason score.
