Objective: To study the correlation of the dynamic changes of serum vascular endothelial growth factor(VEGF)with the severity and prognosis in patients with acute cerebral infarction(ACI).Methods: 46 cases with first ACI who were diagnosed by CT or MRI and admitted to the hospital within 24 hours in our department of neurology were collected as the experimental group.The levels of VEGF in the serum of the patients were measured by double antibody sandwich enzyme-linked immunosorbent assay(ELISA)in the early morning of 1,3,7 and 14 days.At the same time,according to the National Institutes of Health Stroke Scale(NIHSS),the patients were divided into three groups: mild,moderate and severe.The patients were reevaluated 14 days after the onset of the disease.If the NIHSS score was lower than or equal to 4 points at the time of admission,or the score was less than 4 at the time of admission and the score did not increase after reassessment was defined as the group with good prognosis,whereas the group with poor prognosis.In addition,15 healthy people with age and gender matched with no abnormal results of head CT or MRI were selected as the control group to detect normal serum VEGF.Then to compare the difference of serum VEGF level among groups.The data were analyzed by SPSS 25.0.Results:1.Compared with the control group,the level of serum VEGF in ACI group was significantly higher on the 1st,3rd,7th and 14 th day(P < 0.001).And it reached the peak on the 7th day after ACI,almost three times of the control group.It decreased on the 14 th day after ACI,however,there was still significant difference between the two groups(P < 0.001);2.The serum VEGF level of ACI patients has a tendency to increase with the severity of the disease at different time points.But except for 14 days after the onset(P<0.05),there was no statistically significant difference in serum VEGF levels among the three groups of mild,moderate and severe stroke(P > 0.05);3.The serum VEGF level of ACI patients in poor prognosis group showed a higher trend than that of good prognosis group at each time point,but there was no statistical difference between the two groups(P > 0.05).Further subgroup analysis,in the same severity of stroke,the serum VEGF level of poor prognosis group showed a higher trend than that of good prognosis group at different time points,but there was no significant difference between the two groups(P > 0.05),except the first day of the mild stroke group(P < 0.05).And this phenomenon did not appear in the moderate stroke group.4.There was a positive correlation between serum VEGF level of the patients with ACI at 3,7 and 14 days after onset and NIHSS score at admission,the correlation coefficients are 0.31?0.35?0.36.There was no correlation between the two on the first day.Conclusion:1.The serum VEGF levels increased after the occurrence of ACI,and it fluctuated with the progression of ACI;2.The serum VEGF level is related to the severity of ACI;3.There is a trend of correlation between serum VEGF level and short-term functional prognosis.
