Objective:To investigate the diagnostic value of prostate transabdominal B-ultrasonography in evaluating male bladder outlet obstruction.Method: 60 male hospitalized patients with lower urinary tract symptoms(LUTS)from January 2016 to November 2017 were retrospectively analyzed.The left and right diameters,anterior and posterior diameters,upper and lower diameters of prostate were measured by transabdominal ultrasound,and the ratio of left and right diameters(R1),upper and lower diameters and anterior and posterior diameters(R2),ratio of anterior and posterior diameters and left and right diameters(R3)were calculated.At the same time,prostate volume(PV)was calculated.Bladder obstruction index(BOOI)was recorded by urodynamics detector,and bladder outlet obstruction was judged as BOOI > 40(BOO).These data can be divided into two panels,refering to BOOI>40 and BOOI?40 as the dividing criteria,then compare the statistical differences of the above parameters between the two sorts.Analysis of its correlation with the length and proportion of prostate segments and prostate volume;Aim to screen the influencing factors of BOOI in LUTS patients by means of correlation analysis;In the end,a receiver operating characteristic curve(ROC)was established to evaluate the diagnostic value of each parameter in predicting BOO in LUTS patients.Results :In this study,39 of the 60 subjects were diagnosed with BOO,while the number of undiagnosed cases was 21,with a calculated diagnosis rate of 65%.BOOI presented moderately correlated with upper and lower diameters,right and left diameters,volume,and ratio of left and right diameters to upper and lower diameters,while front and rear diameters,upper and lower diameters to front and rear diameters,and front and rear diameters to right and left diameters were low or not correlated with BOOI.The AUC of area under ROC curve for volume prediction BOOI>40 is the largest,reaching 0.943,the best boundary value is greater than 52.62 mL,sensitivity is 89.74%,specificity is 85.71%,95% confidence interval is 0.851-0.986,P<0.0001.Conclusion: It is rationally to draw a summary that B ultrasound parameters have high diagnostic value for BOO patients,which can be used as a supplement to urodynamic parameters,but still cannot completely replace urodynamic examination.
