Professor Zhang Chao's Experience In Syndrome Differentiation And Treatment Of Liver Cirrhosis And The Digging And Analysis Of The Prescription Of Its Prescriptions

Objective: Based on the clinical medical history data of Professor Zhang Chao,a famous Chinese medicine doctor in Yunnan Province,using data mining technology to analyze and summarize the prescriptions and clinical experience of the prescriptions for the treatment of liver cirrhosis in order to further guide clinical practice.Methods: By collecting and sorting the medical records of cirrhosis diagnosed and treated by Professor Zhang Chao from 2014 to 2019,215 cases of clinical medical records that meet the requirements were standardized and sorted,and entered into the "TCM inheritance assistance platform V2.5" software,and the medical record data Perform a systematic analysis.Use the "statistical report" system of the software to carry out the medical records of Chinese and Western medical names,syndromes,symptoms and signs,the main points of syndrome differentiation,prescriptions,and the frequency of the medicinal taste;the data analysis system uses association rules to analyze,New prescriptions and symptom analysis based on entropy clustering of complex systems,as well as symptom-syndrome-drug association analysis of various syndromes,mining the networked display of results,and discussing and analyzing the research results based on TCM theory,summarizing Professor Zhang Chao 's treatment of liver Clinical experience of sclerosis.Result:1.This study included 66 cases,215 cases of clinical data,including 38 males(58%)and 28 females(42%),aged 12-85 years old,32 people in the 40-60 year old group,the largest number of cases,Secondly,there are 24 people in the ?60 year old group,and 10 people in the ?40 year old group.2.The study found that among the 215 medical records,the top 10 symptoms of cirrhosis patients were: abdominal distension,fatigue,fatigue,fatigue,dry mouth,lower extremity edema,loose stools,yellow urine,bitter mouth,ascites.The tongue is mainly composed of red tongue and dark red tongue,and the sublingual tongue veins are more tortuous.The tongue fur is yellowish greasy and white greasy fur.The pulse is mainly composed of thin pulse,smooth pulse string and pulse string.Analysis of symptom association rules resulted in 5 association rules.Based on unsupervised entropy clustering method integration analysis,15 new symptom combination patterns were obtained.3.Statistical results of the names of traditional Chinese medicine syndromes,including four names of traditional Chinese medicine including bulging,accumulation,flank pain,and jaundice;syndrome statistics are sorted in order of frequency: damp heat accumulation syndrome,spleen and kidney yang deficiency syndrome,water and damp internal syndrome Liver Qi stagnation syndrome,liver and kidney yin deficiency syndrome,blood stasis obstruction syndrome.4.The main medications are those with warm sex,sweet taste,spleen,stomach and liver meridian,which have the effect of strengthening the spleen and qi.A total of 142 traditional Chinese medicines were used,and the node threshold> 80 was used to obtain 17 flavors of key medicines,which were basically consistent with high-frequency medicines.The analysis of drug association rules shows that Professor Zhang Chao's most closely-related drug combination is "Kugen-Chishao" in the treatment of liver cirrhosis,as well as the combination of Astragalus,Astragalus,Radix Atractylodes,Poria and other drugs with Chishao.When the degree of support is 20% and 40%,the higher correlation is "turmeric-red peony" and "curcuma-red peony".The core drug combination is "Astragalus,Astragalus,Radix Atractylodes,Poria,Red Peony,Pueraria Root,Jiaoshenqu,Amomum,Licorice",and the new prescription analysis has obtained 9 candidate new prescriptions.5.Syndrome-syndrome-drug correlation analysis separately obtained high frequencyof various syndrome types of damp heat syndrome,spleen and kidney yang deficiency syndrome,water and damp internal resistance syndrome,liver qi stagnation syndrome,liver and kidney yin deficiency syndrome,blood stasis syndrome.Symptoms,symptom rules and symptom-syndrome networks,high-frequency drugs and drug-syndrome networks,symptom-syndrome-drug network displays.Conclusion: Through the application of Chinese medicine assistant platform system,the clinical data of Professor Zhang Chao's diagnosis and treatment of liver cirrhosis were analyzed and studied.Professor Zhang Chao's principles for the treatment of liver cirrhosis are as follows:(1)Diagnosis and treatment characteristics: heart and soul,combining disease and syndrome,syndrome differentiation and treatment.In the diagnosis and treatment of cirrhosis,under the premise of adhering to the guidance of traditional Chinese medicine theory,we should pay attention to the complementary advantages of integrated traditional Chinese and Western medicine,and emphasize the organic combination of disease differentiation and syndrome differentiation.(2)Basic treatment rules: Strengthen the spleen and soften the liver,promote blood circulation and remove blood stasis.According to the pathogenesis characteristics of liver spleen disorder "Liver and spleen disorders,Zhengxu blood stasis",Professor Zhang Chao established the basic treatment of "spleen and soft liver,promoting blood circulation and removing blood stasis".To strengthen the spleen,we must first strengthen the body,promote blood circulation to remove blood stasis,soften the liver,and reconcile liver and spleen function.The medicine features are:(1)The core prescription "Jianpi Ruangan Decoction" is added and subtracted according to the certificate.Recipes of astragalus,radix ginseng,tonifying yin and yin,fried atractylodes rhizome,spleen,spleen and dampness,red peony root,promoting blood circulation and removing blood stasis,pueraria root,promoting yang collaterals,divine qu,amomum spleen and stomach,licorice blending various medicines Either plus green leaf gall and Yinchen;spleen and kidney yang deficiency syndrome plus aconite and other warm yang medicines;water anddamp internal resistance plus polyporus,Alisma,water pomegranate,etc.;liver qi stagnation syndrome plus blue flower ginseng,spider fragrance,etc.;Liver and kidney yin deficiency syndrome plus Ligustrum lucidum,Cuscuta chinensis,Schisandra chinensis,and other drugs;blood stasis blocking syndrome plus cockroach,Angelica,Panax,etc.(2)Combination of Chinese medicine and national medicine.Yunnan's ethnic medicine resources are rich,and various ethnic minorities have accumulated valuable experience in the prevention and treatment of diseases in the practice of production and life.Professor Zhang Chao is good at combining Yunnan national medicine and traditional Chinese medicine for the treatment of chronic liver diseases such as cirrhosis.Compatibility and application of traditional Chinese medicines such as green leaf gall,red ginseng,vines,cockroaches,spider incense,etc.in traditional Chinese medicine prescriptions have good clinical efficacy.However,the clinical application of ethnic medicine to prevent and treat cirrhosis has not yet been developed and utilized,and it is worth exploring in depth.The application of data mining methods provides new ideas and new methods for the inheritance of the experience of famous and old Chinese medicine,which is helpful for the learning and improvement of young doctors and is worthy of application and promotion.
