Policy Research On Community Family Doctor System From Public Perspective

In recent years,with the increasing demand for medical and health care,the aging of the social population,the uneven distribution of medical resources,and the tense relationship between doctors and patients,the common people feel that "large hospitals are too expensive,small hospitals are not well treated" and other issues are increasingly obvious.In order to solve these problems and do a good job in people's health protection,in the new round of medical reform in China,the community family doctor system for the purpose of hierarchical diagnosis and treatment is taken as an important measure of the new round of medical reform.Community family doctor system is one of the national public policies which has been promoted in recent years.The family doctor system is to provide reasonable,long-term,continuous and high-quality medical services for the contracted residents by signing contracts with the family doctor team as the main body of service,so as to eventually achieve graded diagnosis and treatment,optimize the medical environment in China,and solve the medical problems for the common people.After trial cities such as Shanghai,Guangzhou and Hangzhou,this policy entered the stage of full implementation of community family doctor system in 2016.Through the gradual implementation of the community family doctor system,the health and chronic disease management of the elderly has achieved results,initially alleviated the contradictions between doctors and patients,optimized the medical environment,and laid a solid foundation for hierarchical diagnosis and treatment.In the face of major disease outbreaks,through the reasonable triage of community family doctors and the medical mode of shunting patients,the medical resources are allocated efficiently and reasonably,which fully reflects the scientificity and importance of community family doctor system.Community family doctor system is the most scientific and effective medical reform system in China.This paper takes Wolong District of Nanyang City,Henan Province as the research object,through questionnaire survey,interview and other methods,to investigate the community family doctor system in Wolong District of Nanyang city.Through the research,the author concludes that: the community family doctor system has been fully implemented in the District,the residents of the district have recognized the community family doctor system to a certain extent,and the construction of the community family doctor system team has achieved initial results;at present,Wolong District of Nanyang city has completed 225000 contracted residents,established 184 family doctor teams,23 community medical service centers,and special poor households The contract signing rate of family doctors was 92%,which was satisfied by residents in the district in terms of hypertension,diabetes and other chronic diseases management.Through the implementation of the community family doctor system,the medical treatment process in the district was optimized,which effectively eased the doctor-patient relationship,reasonably allocated medical resources,and achieved initial results in hierarchical diagnosis and treatment.Through the research,the author found that there are still some deficiencies in this area: the publicity of the community family doctor system is not in place,the residents have biased cognition of the family doctor system,and it is difficult for the family doctors to sign a contract;the family doctors are short of talents,and the social status and income of the family doctors do not match the actual work;the residents have not implemented the first consultation system of the family doctors after signing the family doctors,and the family doctors policy has been implemented There is no two-way referral in the process,and ultimately no comprehensive and graded diagnosis and treatment.In view of these problems,through quantitative and qualitative analysis and research,drawing on the experience of some domestic cities and foreign community family doctor system,combined with the theory of public policy,the following suggestions are made:(1)community family doctor system is a scientific and reasonable medical model recognized by the world,and improving the family doctor system is the necessary condition to achieve the effect of the policy.(2)Improve the signing rate of family doctor services,so that the majority of residents can enjoy the benefits of the policy,which is more conducive to the effective promotion of the long-term policy.(3)We will continue to promote hierarchical diagnosis and treatment and strengthen the policy objectives of the new medical reform.(4)We should pay attention to the details of policy implementation through the way of humanized contract signing and the establishment of demonstration community.
