Obiective:To investigate the changes and clinical significance of humoral and cellular immunity in children with severe Mycoplasma pneumoniae pneumonia.Methods:200 cases of Mycoplasma pneumoniae pneumonia(MPP)children admitted to the Fourth Affiliated Hospital of China Medical University from October 2018 to October 2019 were collected.They were divided into severe MPP group and ordinary MPP group.There were 73 cases in the severe MPP group,including 36 males and 37females,aged 2-14 years,with an average age of 5.90±2.21 years,5 cases<3 years,and68 cases?3 years.There were 127 cases in the ordinary MPP group,including 66 males and 61 females,aged 1-14 years,with an average age of 5.21±2.94 years,20 cases<3years,and 107 cases?3 years.Serum CRP,serum iron and immune indexes IgA,IgG,IgM,CD3~+,CD4~+,CD8~+and CD4~+/CD8~+were compared.Results:1.CD3~+(1597.57±880.22/microliter),CD4~+(796.77±440.46/microliter)and CD4~+/CD8~+(1.02±0.29)were all lower in the severe MPP group than in the normal M PP group CD3~+(2056.77±1034.95/microliter),CD4~+(1035.03±576.39/microliter),an d CD4~+/CD8~+(1.19±0.41),with statistically significant differences(P<0.05).There was no statistically significant difference between CD8~+(777.27±367.88/microlite r)and ordinary MPP group(888.99±439.89/microliter)(P>0.05).2.IgM(1.48±0.55g/L)and IgG(9.79±2.44 g/L)in severe MPP group were highe r than that in normal MPP group IgM(1.33±0.46g/L)and IgG(9.03±2.43g/L),w ith statistically significant differences(P<0.05).IgA(1.39±0.66g/L)was higher than that in normal MPP group(1.26±0.68 g/L),but no statistically significant differen ces(P>0.05).3.IgM(1.46±0.48g/L)and IgG(9.99±2.38g/L)in severe MPP group?3 yearsol d were higher than those in normal MPP group(1.32±0.45g/L)and IgG(9.26±2.38g/L),with statistically significant differences(P<0.05).The IgA(1.44±0.66g/L)l evel of children in the severe MPP group was not significantly different from that in the normal MPP group(1.34±0.67g/L)(P>0.05).Serumlevels of IgA(0.70±0.27g/L),IgM(1.76±1.27g/L)and IgG(7.03±1.35g/L)in children with severe MPP<3 years old were not statistically significant compared with IgA(0.85±0.61g/L),Ig M(1.35±0.48g/L)and IgG(7.82±2.39g/L)in the same age of normal MPP groups(P>0.05).4.Serum CRP level in severe MPP group was significantly higher than that in normal MPP group,with a statistically significant difference(P<0.05);serum iron level in severe MPP group was lower than that in normal MPP group,with a statistically significant difference(P<0.05).Conclusions:The cellular immunity(CD3~+,CD4~+,CD4~+/CD8~+)and humoral immunity(IgA,IgG,IgM)of children with severe MPP had obvious functional disorders,and the humoral immunity disorders had age differences.At the same time,the children also had changes in serum CRP and serum iron.Therefore,combined detection of humoral immunity,cellular immunity,serum CRP and serum iron in children is conducive to early identification of severe MPP.
