Objective:To investigate the effects of Danhong injection on the expression of BMP-2,7 and b FGF during early healing of rib fracture,and to explore the role and mechanism of BMP-2,7 and b FGF in inducing osteogenesis.Method:This study collected patients who were hospitalized in the emergency trauma surgery or thoracic surgery of Putuo Clinical Medical College affiliated to Shanghai University of Traditional Chinese Medicine from May 2017 to May 2018.After screening,80 patients with standard rib fractures were included in the database.A prospective randomized controlled trial randomized 80 patients with rib bone into 2 groups.The treatment group were received conventional western medicine+Danhong injection?0.9%Na Cl 250ml+Danhong injection 20ml,2 times/day?,control group were given regular western medicine.Conventional western medicine treatment includes external analgesia,anti-infection,dehydration and swelling treatment,and symptomatic support treatment such as nebulization and oxygen inhalation.The VAS pain scores?status in rest and cough?,hospital days,serum ALP concentration,serum Ca2+,P5+concentration were observed in the Danhong injection treatment group and the non-danhong injection treatment group at the time of admission and on the 7th day after treatment.TCM syndrome score,TCM syndrome effect,BMP-2,7 content changes,statistical analysis,were used to evaluate Danhong injection on bone morphogenetic protein in patients with early rib fracture.Result:1.There was no statistical difference in clinical baseline data between the two groups?P>0.05?.2.Compared the VAS scores between the treatment group and the control group at the time of cough and rest.The results showed that there was no significant difference in VAS score between the two groups at the time of admission?P>0.05?,but after 3 days of receiving the corresponding drug treatment,the two groups VAS scores showed a downward trend,and the difference in statistical comparison was significant?t=-4.611,P<0.05 at the time of cough;t=-1.856,P<0.05 at rest?.And the two groups were compared after 7 days of treatment,there was also a significant difference in VAS scores?t=-10.977,P<0.05 at cough;t=-3.506,P<0.05 at rest?,indicating a significant reduction in VAS scores after 7 days of treatment with Danhong injection.3.After 7 days of treatment,the two groups were analyzed as follows:the average length of hospital stay in the treatment group?12.71±4.74?,and the average hospital stay in the control group?9.98±2.30?.The difference between the two groups was statistically significant?P<0.05?,indicating that Danhong injection can effectively reduce the number of hospital stays in patients with rib fractures.4.At the time of admission,there was no significant difference in serum calcium ion concentration between the two groups?P>0.05?.After 7 days of treatment,the calcium concentration in the treatment group and the control group increased significantly?t=-5.342,P<0.05?.;t=-3.290,P<0.05),but when the two groups of data were compared,the results showed that the calcium level of the treatment group was significantly higher than that of the control group?t=2.573,P<0.05?,indicating that Danhong injection treatment has a significant effect on the increase of calcium ion concentration;At the time of admission,there was no significant difference in serum phosphorus ion concentration between the two groups?P>0.05?.After 7 days of treatment,the phosphorus ion concentration in the treatment group was not significantly decreased?t=0.995,P>0.05?,while there was a significant decrease?t=2.634,P<0.05?of the phosphorus concentration in the control group before and after treatment.But when the two groups of data were compared,the results showed no significant difference in serum phosphorus ion concentration between the two groups?Z=-1.011,P>0.05?.The decrease of phosphorus ion was more significant in the control group,indicating that Danhong injection had no significant effect on the reduction of phosphorus concentration in the treatment of rib fracture.5.At the time of admission,the serum of two groups of patients was taken and the ALP content was determined.The statistical results showed that there was no significant difference in ALP levels between the two groups?Z=-1.670,P>0.05?.And serum samples were taken again after 7 days of corresponding drug treatment.The serum ALP levels in the two groups were significantly higher in the treatment group and the control group before and after treatment?t=-8.172,P<0.05;t=-2.488,P<0.05?.While the two groups of patients were treated after 7 days,the results showed that the ALP content in Danhong injection group was significantly higher than that in the control group?t=3.940,P<0.05?,indicating that Danhong injection had a significant effect on the increase of ALP content in the treatment of rib fracture.6.At the time of admission,the serum of two groups of patients was taken to determine the content of BMP-2.The statistical results showed that there was no significant difference between the two groups?t=-0.629,P>0.05?.After treatment for 7 days,the two groups were analyzed.Before and after treatment,the BMP-2 levels in the treatment group and the control group were significantly increased?Z=-7.698,P<0.05;t=-5.254,P<0.05?.And the results of statistical analysis showed that the increase of BMP-2 in Danhong injection group was more significant than that in the control group?Z=-7.544,P<0.05?,indicating that Danhong injection has a significant effect on the increase of BMP-2 content in the treatment of rib fracture.7.At the time of admission,the serum of the two groups were taken and the BMP-7content was determined.The statistical results showed that there was no significant difference between the two groups?t=-1.453,P>0.05?.After treatment for 7 days,the BMP-7 levels in the treatment group and the control group were significantly increased before and after treatment?t=-14.438,P<0.05;t=-7.761,P<0.05?.After 7 days,the results of statistical analysis showed that the BMP-7 level in the Danhong injection group was significantly higher than that in the control group?t=-6.465,P<0.05?,indicating that Danhong injection has a significant effect on the increase of BMP-7 content in the treatment of rib fracture.8.At the time of admission,the serum of the two groups were taken and the b FGF content was determined.The statistical results showed that there was no significant difference in b FGF between the two groups?Z=-0.880,P>0.05?.After 7 days of treatment,the b FGF content of the treatment group and the control group were significantly increased before and after treatment?t=-7.207,P<0.05;t=-9.884,P<0.05?,and the increase of b FGF content in Danhong injection group was significantly higher than that in the control group?t=6.043,P<0.05?,indicating that Danhong injection has a significant effect on the increase of b FGF content in the treatment of rib fracture.9.At the time of admission,the TCM syndrome scores of the treatment group and the control group were not significantly different?P>0.05?.After 7 days of the corresponding drug treatment,the scores of the Danhong injection group were significantly lower than the control group?Z=-2.243,P=<0.05?,indicating that Danhong injection can significantly reduce TCM syndrome scores when treating rib fractures.10.The total effective rate was 92.9%in the Danhong injection group and 81.6%in the control group.The difference in the total effective rate of TCM syndrome between the two groups was statistically significant?P<0.001?,indicating that the treatment group was more effective than the control group.Conclusion:1.Danhong injection can safely and effectively improve the clinical symptoms and signs of patients with rib fractures,shorten the length of hospital stay,reduce the VAS pain score,and significantly improve the efficacy of TCM symptoms.2.The mechanism of Danhong injection promoting fracture healing is related to the promotion of BMP-2,7 and b FGF factors.
