Objective: To explore the effect of family therapy in improving rural bereavement depression,family function,coping styles,happiness.to promote scientific evidence for improving the mental health of bereaved families in rural areas.Method:This research used a randomized controlled trial design and a semi-structured psychological interview for pre-treatment and post-treatment evaluation.Total of 36 patients who met the requirement were randomly divided into family therapy group(n=18)and routine treatment group(n=18)from 2 villages and towns in a county-level in Liaoning province.family therapy group was implemented 10 sessions family therapy,routine treatment group have no intervention.The research tool use the self-compiled general social demographic questionnaire,Hamilton Depression Scale(HAMD),Structured Clinical Interview for DSM-IV(SCID),Family Assessment Device(FAD),Grief Experience Questionnaire(GEQ),Coping Style Questionnaire(CSQ),Chinese Happiness Inventory(SCHI)to assessment effect of family therapy.Data settle,analysis and processing were used SPSS22.0.Result:(1)After intervention,function of family therapy group among problem solving(t= 5.019,p = 0.000),roles(t =-2.516,p = 0.16),communication(t =-5.268,p = 0.000),affective response(t =-5.268,p = 0.000),affective involvement(t =-3.582,p = 0.001),behavior control(t = 3.225,p = 0.003),general function(t =-4.706,p = 0.000),seven dimensions post-treatment scores significantly lower than pre-treatment scores,there are no significant differences in routine treatment group,except problem solving(t =2.170,p = 0.045),and post-treatment scores in family therapy group among problem solving(t =-5.019,p = 0.000),roles(t =-2.516,p = 0.016),communication(t =-5.268,p = 0.000),affective response(t =-5.268,p = 0.000),affective involvement(t =-3.582,p = 0.001),behavior control(t =-3.225,p = 0.003),general function(t =-4.706,p =0.000)are significantly lower than post-treatment scores in the routine treatment group.(2)After intervention,in family therapy group,post-treatment of depression total scores(t = 5.848,p = 0.000)scores are significantly lower than pre-treatment scores,there are no significant differences in routine treatment group,and post-treatment scores in family therapy among depression total scores(t =-2.114,p = 0.043)are significantly lower than the post-treatment scores in routine treatment group.(3)After intervention,in family therapy group,post-treatment of grief experience total scores(t=8.152,p=0.000)are significantly lower than pre-treatment scores.In routine treatment group,post-treatment of grief experience total scores(t=3.134,p=0.006)are significantly lower than pre-treatment scores.The post-treatment scores in family therapy among grief experience total scores(t=-3.237,p=0.003)scores are significantly lower than the post-treatment scores in routine treatment group.(4)After intervention,in family therapy group post-treatment scores of coping style among solving the problem(t =-2.543,p =0.000),help(t =-2.631,p = 0.018)are significantly higher than pre-treament scores,post-treatment scores among avoid(t = 2.412,p = 0.027),rationalization(t = 2.334,p =0.032),self-accusation(t = 4.740,p = 0.000),fantasy(t = 2.435,p = 0.026)are significantly lower than pre-treatment scores,there are no significant differences in routine treatment group,and post-treatment scores in family therapy group among solving the problem(t = 2.308,p = 0.027),help(t = 2.942,p = 0.006)are significantly higher than post-treatment scores in routine treatment group,post-treatment scores in family therapy group among self-accusation(t=-2.235,p=0.033)is significantly lower than post-treatment scores in routine treatment group.There are no significant differences in other dimensions.(5)After intervention,in family therapy group post-treatment scores of happiness among optimistic(t =-5.036,p = 0.000),social commitment(t=-2.122,p = 0.049),positive affect(t =-5.720,p =0.000),satisfaction with self(t =-3.828,p = 0.001),achievement at work(t =-2.500,p =0.023)downward social comparisons(t =-4.267,p = 0.001),peace of mind(t =-5.762,p= 0.000),happiness total scores(t =-5.562,p = 0.000)are significantly higher thanpre-treament scores,post-treatment scores among physical fitness(t =-1.567,p = 0.135),sense of control(t=-2.026,p=0.059)are no significantly difference.There are no significant differences in routine treatment group,and post-treatment scores in family therapy group among sense of control(t = 2.396,p=0.022),peace of mind(t = 3.820,p =0.001),optimistic(t = 3.578,p = 0.001),social commitment(t=4.224,p=0.000),positive affect(t = 5.442,p = 0.000),satisfaction with self(t = 3.599,p = 0.001),happiness total scores(t = 4.720,p = 0.000)are significantly higher than post-treatment scores in routine treatment group,there are no significant differences in other dimensions.(5)The mediation test show that family function play a complete mediating role between family therapy and depression.Conclusion:(1)Family therapy can significantly improve the family function,depression degree,coping style and happiness of bereaved survivors in rural China.(2)Mediation effect shows that family therapy improves depression by improving family function.(3)Future family therapy intervention for bereaved survivors in ruaral China suggestions: the key to improve the function of family that establish "the time bank",psychological prevention,encourage raising nominal kinship,support mutual-groups,popularize mental health knowledge way to promote the mental health level of bereaved survivors in rural China.
