Research On The Application Of Personal Care Planning Model In The Medical Service System Of Aging And Retirement Institutions

Since entering the 21 st century,the problem of global aging has been highlighted.China has even entered an aging population society.Various phenomena indicate that the elderly population in China will grow at a relatively rapid rate.The long-term care for the elderly has become a very important social hot issue.Different from the traditional pension service model,the medical and healthcare integration pension service model,while inheriting the traditional medical and pension services,integrates well the existing medical service and pension service resources,and promotes the development of the medical care integration service model.Aggressive aging needs to plan and integrate relevant pension systems to meet the urgent needs of an aging population.Therefore,it is imperative to build a long-term care service system that is oriented toward service demanders.The model of personal care planning refers to a team consisting of doctors,nurses,social workers,caregivers,dieticians,and rehabilitators in the framework of the overall care and integration model.Personal needs of personal care service plans and implementation of services.The personal care plan model further innovated the care mechanism for the medical care and integration of the aged care institutions,providing elderly people with comprehensive and integrated quality care.The G City Home for the Aged is a public pension institution that integrates medical care,medical care,nursing,rehabilitation,social work,and nutrition.It is dedicated to providing professional and high-quality care services for the elderly.Since April 2015,G City Homes for the Aged has begun to implement a personal care plan service model.At present,personal care plans have been fully implemented in all parks in the G-House of the elderly.Therefore,the analysis of the G city nursing homes as a case can comprehensively and specifically understand and explore the application of the personal care planning model in the service system of the medical care and integration pension institution.In view of this,first of all,this study summarized the current domestic and foreign research results related to the personal care planning model through literature analysis,and systematically combed the existing research theories;secondly,the study was conducted on the basis of G City homes for the elderly.To observe the model of personal care plan for the elderly homes in G city,conduct interviews and satisfaction questionnaires for the participants of the personal care plan model,conduct interviews with cross-professional team staff,and survey questionnaires with satisfaction.The implementation process of the personal care plan model in the service system of the medical care and nursing care home care institutions explored the effectiveness of the implementation of the personal care plan model.Finally,this study analyzed in depth the process of embedding the personal care plan model in the service system of medical care and nursing care institutions.Based on the difficulties and their causes,and on this basis,they put forward some targeted countermeasures and suggestions in terms of systems,management,and services,with a view to better promoting the personal care planning model in the service system of medical care and nursing care institutions.Effective application.
