A Dynamic Study On The Satisfaction Of Long-term Care Needs Of Disabled Elderly Residents And Its Influencing Factors

In 2016,the number of disabled elderly in China has reached 40.63 million,accounting for 18.3% of the total elderly population.Under the background that the scale of disabled elderly in China is huge and the long-term care system has not yet been established,is the long-term care of disabled elderly in China satisfied? What are the factors that influence the satisfaction of long-term care needs of incapacitated elderly under the condition that diversified care resources such as family,community,market and country can be fully utilized? This is the concern of this study.The study of this problem can provide an intervention path for improving the quality of life of disabled elderly people and put forward targeted Suggestions for the institutionalization of China's long-term care system.This study reviews the relevant literature on long-term care of disabled elderly,combines the hierarchy of needs theory,welfare pluralism theory and social support theory,and studies the satisfaction of basic care needs of disabled elderly at home based on survey data.In the empirical part,the data of CLHLS in 2005,2008,2011 and 2014 are used to show the dynamic changes of the long-term care needs of the disabled elderly at home.Using binary logistic regression model,the factors influencing the satisfaction of the care needs of the disabled elderly at home are analyzed from three social support subjects: community,society and government.The study found that:(1)The care needs of the disabled elderly in our country are not satisfied for a long time,and the degree of satisfaction of the disabled elderly care needs shows obvious differences between urban and rural areas,regions,and gender.(2)The economic situation,the degree of disability,and the caregiver's performance have always had a significant impact on the care needs of the disabled elderly at home.(3)At the present stage,the main body of social support for long-term care of the disabled elderly in China mainly comes from the family.The long-term care needs of the disabled elderly,who are mainly cared for by their families,are consistently better met than those of social resources.(4)The provision of care services by the community has a significant positive impact on the satisfaction of long-term care needs of the disabled elderly at home.The care needs of home disabled older persons with care resources in the host community are more satisfied than those of disabled persons without care resources in the community.There is an increasing trend in the need for community care for the disabled elderly at home,with an increasing number of disabled older persons looking to the community for community care services.Although the proportion of the country's communities providing care services is currently increasing year by year,it is not yet able to meet the needs of disabled older persons.(5)The improvement of the social security system has a positive impact on the satisfaction of the long-term care needs of the disabled elderly in the home.With the socio-economic development of our country,the coverage of social security has been expanded,and the proportion of disabled elderly people with Social Security has been increasing year by year.The gap between the care needs of socially insured disabled persons in the home and those of disabled persons without social security is widening.Based on the research findings,the following policy suggestions are put forward:(1)The national level should improve the social security system and speed up the implementation of the long-term care insurance system;Development of family care support policies;We will narrow the differences between urban and rural areas and between regions.(2)At the market level,we need to cultivate professional care personnel.(3)Community care services should be vigorously developed at the community level.(4)At the family level,we should promote filial piety and foster respect for and love of the elderly.
