The population aging in the world is increasingly showing the trend of aging and aging.The increase of the average life expectancy of the elderly naturally increases its health risks,which has led to a series of problems of elderly health.Among them,the most prominent problem is the elderly's disability,impaired and long-term care problems.In order to meet the long-term care needs of the elderly,many developed countries have already explored the establishment of the elderly care insurance system.China has gradually realized in recent years the seriousness of the problem of elderly care support and formally put it on the agenda during the "13th Five-Year Plan".Shanghai,as an international metropolis and one of the cities with the most aging population in China,faces the major problems.As the population is aging,longevity,and disability,the proportion of the elderly with semi-disabled increases year by year.The requirements of the elderly for long-term care is constantly increasing.However,with the development of society and the economic transformation,the family retirement function has been gradually weakened.In addition to the traditional community-based single-family pension model,monotonous service forms and content,and the high cost of specialized nursing organizations,the elderly in the region have long-Nursing needs are difficult to fully meet.Based on this,it is very important to set up a relatively perfect elderly long-term care insurance system as soon as possible and choose a suitable development model for the region.At present,the pilot "Long-Term Care Insurance" in Shanghai has not yet formed a sound development model.It mainly depends on the basic medical insurance.The fund-raising mainly involves the transfer of Medicare Fund plus part of the unit payment.This model obviously has obvious disadvantages and it gradually Increasing the pressure on payment of Medicare funds is difficult to sustain in the long run.Therefore,how to choose a long-term care insurance model suitable for the development of Shanghai,determine the financing mechanism of insurance fund and the corresponding payment level,and rationally design the rate-sharing ratio of government,individuals and insurance beneficiaries are the problems we must solve.This dissertation is divided into eight chapters to study the construction of Shanghai elderly care insurance model.The first chapter introduces the research background and significance of the research,the status quo of academic circles at home and abroad and the research methods and innovations used in this paper.The second chapter Defines the related concepts and theories involved in this article,and lays a theoretical foundation for the combing of the major nursing insurance modes in China and other countries and the research on the construction of "long-range insurance" mode in Shanghai.The third chapter discusses the current supply and demand status of the elderly care in Shanghai As well as the current necessity of establishing Shanghai's long-term care insurance for the elderly in Shanghai.Chapter Four reviews the current three major models of elderly care insurance at home and abroad,and provides a comparatively analysis of the major modes for providing empirical reference for Shanghai's model selection.The fifth chapter puts forward the idea of constructing long-term care insurance mode in Shanghai combining with the current pilot practice in Shanghai and designs its financing mechanism and payment mechanism.Chapter 6 takes the long-term balance of financial supply and demand of long-term care insurance as the starting point to calculate its insurance revenue and expenditure.In the funding mechanism,this paper proposes that the government,individuals and beneficiaries of insurance pay the tripartite rate.The ILO financing model is first constructed to measure the total amount of elderly care needs in Shanghai.The total cost of care insurance needs to be determined according to the total demand for nursing care in the future.The pay-as-you-go financing model is designed to determine the overall level of treatment of the system.The concept of multi-party sharing of social insurance in the financing model is shared by governments,individuals and insurance users.In this paper,by quantitative methods and drawing on advanced experience from abroad,it is reasonable to determine the share of all parties and set them within a reasonable range.Chapter Seven and Chapter Eight propose targeted models for building policy support and prospects for future research.
