Research On The Supply Modes Of Medical-nursing Combined Old-age Care

Since the 21st century,the aging of the population has developed into a global problem.Since China has a large population base,the elderly population grows faster and the aging process is gradually accelerating.The decline of the fertility rate and changes of population structure led to an increasingly miniaturized and core family scale which has weakened the traditional family elderly care function,and the pressure of social endowment has also grown.The lengthening of the average life expectancy makes an increase in the number of elderly,disabled and partially disabled elderly people.The problem of "longevity but unhealth" has become increasingly prominent and the demand for medical services by the elderly is becoming more and more urgent.While the existing old-age care mode,which only provides basic living care is difficult to take good care of the old-age care and medical needs.In order to meet the needs of the elderly in old-age care service and medical service,it is imperative to promote the combination of medical care and old-age care services with integrating medical resources and endowment resources,and provide the elderly with a continuous and professional health service that integrates living care,disease diagnosis,rehabilitation care and health management,to research the goal of improving the well-being of old people in their later life.It is not only a solution to the livelihood issues that touch the nerves of people,but also a long-term plan to promote the reform of China's pension service system and actively respond to the aging of population.The“State Council's Several Opinions on Accelerating the Development of the Elderly Care Service Industry" which was released in 2013 first clearly stated that"actively promoting the combination of medical and elderly care service " shows that China has begun to attach importance to the development of the integration of medical care and old-age care services,and the development of medical-nursing combination have risen from basic concepts to national actions.In order to establish a suitable,rational,comprehensive and continuous medical care system for the elderly under the background of healthy aging,and provide old-age care and medical service for the elderly,we also started construction work of the first and second batch of medical care pilot units in the country respectively in June and September 2016.This paper takes the supply modes of medical-nursing combined old-age care services as the research object.It analyzes that the aging situation of population,such as the rapid growth rate,the structure imbalance,the serious empty nest phenomenon and so on,through the study of the National Statistical Yearbook,China Health and Retirement Longitudinal Study.It gives a review of the existing research results of domestic and foreign scholars,combined with the interpretation and analysis of the theory of public goods,Maslow's hierarchy of needs and the theory of social system.It has a retrospect of the changing trends of old-age care and medical-nursing services and analyzes the motivation of developing the medical-nursing combined old-age care in both the demand side and the supply side.Using non-structured interviews and comparative analysis methods,this paper discusses three different supply modes of medical care and pension service,including the cooperative mode of the pension institution and medical institution of Jiangdu social welfare home in Yangzhou,the mode of the pension institution with a medical room of nursing home in Nanjing and the mode of the pension institution with a comprehensive hospital of Yaoyang elderly apartment in Hangzhou which were visited and investigated by author,and introduced a real case of the Guizhou Yaoyang pension service center relying on the community mode.It found some problems in the supply of medical-nursing combined old-age care,such as the lack of enthusiasm for the participation of medical institutions,the limits of medical insurance policy and the insufficient supply of medical and nursing staff,through the comparison and summary of different supply modes.In order to promote the supply modes of medical-nursing combined old-age care better and improve the accessibility of service,this paper proposed some suggestions to optimize the combination of medical care and pension service,on the basis of exploring the new breakthroughs in the development of medical care and pension service and clarifying the responsibility of the various subjects.The suggestions include perfecting policy and increasing investment to build a scientific institutional system;integrating resources and promoting cooperation to form multi supply of services;supervising strictly and strengthening evaluation to ensure the quality of service supply;innovating technology and cultivating talents to perfect service supporting mechanism.
