Case Study On Improving The Quality Of Life Of Elderly Patients With Chronic Diseases From The Perspective Of Social Support Theory

With the aggravation of aging population in China,the elderly population in China is also increasing rapidly.Older patients with chronic diseases are a common and special group of people in the elderly.Their quality of life has always been concerned.Numerous studies have shown that social support plays an important role in the quality of life,while related intervention studies are rare.Therefore,under the guidance of social support theory,this paper hopes to use social work methods to intervene in the quality of life of elderly patients with chronic diseases,and to explore the impact of social support theory on the quality of life of elderly patients with chronic diseases in the context of social work.This study aims to improve the quality of life of elderly people with chronic disease through case work.From the perspective of social support theory,to improve quality of life of an elderly person with chronic obstructive pulmonary disease(COPD)who has been living in a nursing home in Wuhan.The SF-36(36-item short-form)concise quality of life scale was used to evaluate the current quality of life of the client,and the Social Support Rating Scale compiled by Xiao Shuiyuan was used to evaluate the social support of the client.The intervention in the quality of life of the client through social support is divided into four stages: the observation preparation period,the preliminary construction period,the consolidation period,and the follow-up completion period.By comparing the overall scores and scores in all dimensions of each scales before and after the intervention,and by interviewing and analyzing the client,their families and welfare home staff,etc.the following conclusions were drawn: After intervening in the social support network of the client,the social support network was consolidated and the quality of life improved.In addition,the establishment of the social support network of the case has not only positively affected the quality of life of the client.The author also found that the establishment of social support network in welfare homes has spillover effect,that is,other elderly people in the social support network can also benefit from it,which can be explored by the author in future research.
