Research On The Model Of Long-term Care Service Embedded In Community For Disabled Elderly

With the increase of life expectancy and the change of disease spectrum in China,the absolute number and relative number of disabled elderly groups are increasing.Based on the dual barriers of old age and disability,the ability of the disabled elderly to self-care has gradually weakened,and their dependence on society and others has deepened.This requires continuous innovation in long-term care service models and optimization of service supply networks in response to the growing care needs of the disabled elderly.As a new type of long-term care model,the model of long-term care service embedded in community integrates the relative advantages of home care,community care and institutional care,and it should become an important carrier form of long-term care service supply.Based on the concept definition and frame analysis of the model of long-term care service embedded in community,taking the practice of the model of long-term care service embedded in community in Changsha as an example,the advantages and problems of this model are summarized and countermeasures and suggestions are provided to improve the model of long-term care service embedded in community.The research shows that the number of the disabled elderly who arecovered by the model of long-term care service embedded in community differs in gender,age and household registration,and it has low education,high prevalence of geriatric diseases,and high rates of widowhood.At the same time,the trend of coexistence of aging and disability is obvious.The model of long-term care service embedded in community has a good development trend in practice: the care service subject has a higher willingness to "embed" and the internal construction is more complete;the community embedded platform uses the volunteer resources more efficiently;long-term care service content embedded in community is abundant,the quality of care for the disabled elderly is improved to a certain extent.However,the model of long-term care service embedded in community still faces some problems in practice,such as the insufficient integration of resources of the community embedded platform;the functional embedding mechanism encountering the neighbor avoidance effect;the service delivery network is not sound,and the operation of embedded care institutions is difficult.These problems restricts the effective operation and advantages of the model of long-term care service embedded in community.The key to promote the further development of the model of long-term care service embedded in community lies in improving the external environment and internal mechanism for the model.On the basis of cultivating correct views on care and ageing,in order to improvethe efficiency of long-term care service delivery and the sense of well-being and gain of the disabled elderly,it is necessary to improve the policy environment of the model of long-term care service embedded in community;promote the sustainable operation of community embedded care institutions;strengthen the community's resource integration and governance capabilities,and optimize embedded mechanism and improve the long-term care service delivery network.
