| The problem of aging has appeared all over the world,it poses a great challenge to the supply efficiency of the elderly care services.In this context,‘integrated care’ has become an important concept in the reform of the elderly care service system.Britain is one of the earliest country to develop ‘integrated care’.Through years of practice,Britain formed‘integrated care’ elderly care service system with the characteristic of ‘ put the needs of the elderly as the center,based on community care,effective coordination of medical care and social care as the main path’.This system satisfied the diversity of the elderly pension service demand,also improved the efficiency of the elderly care service suppliers.Now,China is in a state of increasingly severe population aging problem,and the contradiction between the supply and the demand of elderly care services is increasingly prominent,the entire elderly care service system is facing a major challenge.By analyzing the ‘integrated care’ elderly care service system in Britain,we can obtain new ideas and perspectives for the development of the elderly care service system in China and gain better development.Through literature research method,comparative analysis method and case analysis method,this paper deeply and systematically analyzed the ‘integrated care’ system in Britain,and explored the feasibility and localization of the application of ‘integrated care’in China’s elderly service system.On the theoretical basis,complex adaptive system theory and active aging theory provide theoretical support for the analysis of ‘integrated care’ in this paper.Firstly,from the perspective of CAS theory,this paper analyzes the complex adaptability of ‘integrated care’ and the adaptive agents of ‘integrated care’ in Britain.Then this paper analyzes the implementation of ‘integrated care’ in Britain from the perspective of local practice based on the model of complex adaptive system through two practice cases: Aspiring to Clinical Excellence(ACE)and England vanguard sites program.For the feasibility analysis of China,this paper firstly introduces the initial practice of integrated care in China--‘Luohu model’,and summarizes the demand assessment tools in China.Then,through feasibility analysis,this paper comprehensively expounds the advantages and disadvantages of the practice of ‘integrated care’ in China from four perspectives as well as the opportunities and challenges that may be faced.Through the analysis of feasibility,based on the practical experience,this paper summarizes the practice pathway of China to achieve ‘integrated care’ framework,based on thecommunity care and the starting point,as a whole integrated elderly service supply main body,and to establish a perfect coordination mechanism and assessment mechanism,so as to promote the establishment of a comprehensive,balanced and sustainable system for elderly care service,actively respond to the growing aging population service requirements. |
