The Exploration Of The Community Care Model For The Elder With Dementia

With the rapid development of aging in China,the number of elder with dementia is increasing.At the same time,the elder,caregivers and the public request a growing demand for a comprehensive and professional rehabilitation service of dementia.Compared with the developed countries,the development of services for the elder with dementia is lagging behind in our country: the lack of professional social work and a perfect community care model for the elder with dementia,the failure of getting the effective supports of their families,the community as well as the medical technology.This series of problems,in turn,will lead to the increasing contradition between the care for elder with dementia and the society.Therefore,it is an important measure to actively explore the best way of the development of service for dementia elderly and to establish a comprehensive and professional community care service mode as soon as possible.This study,taking the community care project of the elder with dementia in Guangzhou E institution as an example,uses the methods of literature researches,interviews and observations to analyze the theory and practice of community care for the elder with dementia at home and abroad,and uses the theory of ecosystem and the theory of social support to evaluate the ecosystem of elderly with dementia and analyze the objective of the project service.Subsequently,this study summarizes the operation of the community care model for the elder with dementia in the E institution project and refines the current service experience.On this basis,we reflected on the predicament of the community care model of the elder with dementia,and finally try to construct a system of community care services for the elder with dementia,and put forward the countermeasures to improve the community care model of the elder with dementia.
