Purposes: The functional deficits of individuals with chronic ankle instability(CAI)might altered their gait kinematics and kinetics,and the observation of gait characteristics of chronic ankle instability may have clinical significance for the management of CAI.Walking with performing cognitive task has been used as an observation of daily walking posture control and attention distribution,the dual task gait's feature under the intervention of ankle orthoses has not been described in detail.The purpose of this study was to investigate adding the condition of ankle orthoses,i.e.,elastic ankle support,kinesio tape and cognitive task,to explore whether it would change the ankle kinematics and kinetics,as well as the distribution of cognitive resources while walking.Methods: A total of 25 subjects,aged 18-30,who met the criteria for chronic ankle instability proposed by the international ankle association,were recruited.Subjects were required to perform natural gait and dual-task gait experiments under three conditions: bare foot,elastic ankle support and Kinesio tape.The spatial and temporal parameters of the gait and the kinematic and kinetic parameters of the ankle were obtained by threedimensional gait analysis.The difference of the indexes of kinematics and kinetics under different protection conditions was analyzed by using the two-factor repeated measure A-NOVA,with the significance level of 0.05.Results:(1)the walking speed of the subjects in the dual task was 0.095 m/s lower than that in the single task,with statistical difference(P < 0.001).However,the effect of ankle orthoses on subjects' walking speed(P=0.067)was not statistically significant.The loading response period and pre-swing period of the dual-task condition were longer and the toe-off time was later than that of the single-task condition(P < 0.05).(2)there was a statistically significant difference between the number of sitting positions(median of 4.02)and the number of walking with bare feet(median of 3)(P < 0.001),and there was no significant difference in the performance of cognitive tasks under the three conditions of bare feet,elastic ankle support and kinesio tape.(3)different ankle orthoses had no significant effect on the ankle angle during walking,and the performing of the dual task reduced the angular velocity of the ankle at initial contact,maximum plantarflexion angular velocity during loading response,and maximum plantarflexion angular velocity during pre-swing period(P < 0.05).Under the condition of elastic ankle support,the maximum valgus angular velocity in the gait pre-swing period decreased,and the maximum external rotation angular velocity in the load response period decreased(P < 0.05).(4)under the dual-task gait,the first and second peaks of the vertical axis of ground reaction and the sagittal axis decreased(P < 0.05),the maximum dorsiflexion moment during gait loading response period decreased under the conditions of kinesio tape and ankle support(P < 0.05),and there was no difference between the ankle support and kinesio tape.Conclusions:(1)Whether performing cognitive tasks or not,the use of elastic ankle guard reduces the angular velocity of the ankle in walking and provides ankle stability for CAI patients.The kinesio tape had no effect on the kinematics of the ankle during walking,but reduced the flexion moment in the pre-swing period,which may increase the activation degree of the dorsiflexion muscle.(2)In the dual-task gait,the walking speed of CAI patients significantly decreased,which resulted in the change of gait staging proportion,the angular velocity at the moment of ankle contact,the maximum plantar flexion angular velocity during loading decreased,and the first peak value of the vertical axis and sagittal axis of ground reaction decreased.(3)In the condition of dual task,CAI patients' ability to push and accelerate at the end of gait support was weakened,and the function of evertor muscle strength was insufficient.
