Evaluation Of The Clinical Efficacy Of The Method Of Strengthening The Spleen And Reducing The Adverse Effects On The Treatment Of Reflux Esophagitis Of Middle-deficiency-qi Reverse Type

Summary:Reflux Esophagitis(RE),characterized by reflux and heartburn,has become a common and frequent-occurring disease in the digestive system.Since the pathogenesis of RE is of great concern to the weakening of anti-reflux defense mechanism,you could suggest that RE is one of the representative symptoms of synthesizing digestion function decreasing,which also shows as belching,abdominal distension,constipation,as well as secretion reducing of gastric juice and small intestinal fluid.There are a lot of RE patients accompanied with comprehensive dysfunction of digestive system.Studies have shown that gastroesophageal reflux disease(GERD)symptoms alleviated by regular therapy with PPI would relapse rapidly after withdrawal.Traditional Chinese Medicine(TCM)has definite effect in digestive system dysfunction and recurrence,gastrointestinal side effects caused by acid-fast treatment of western medicine could also be effectively prevented by TCM treatment.Therefore,establishment on TCM treatment scheme is a necessity.It could prolong the attack cycle,reduce medicine toxicity and side effects,and improve the quality of life.However,the comprehensively clinical research and evaluation on its curative effect is insufficient,based on a standardized,domestic-and-foreign approved evaluation means and methods.Moreover,there is a lack of a specific treatment-oriented schedule on RE,which requires definite efficacy and replacement possibility of the western medicine.Objective:The aim of this study is to take the unique advantages of traditional Chinese medicine(TCM),and establish an advanced therapy with curative effect,less side effect and low cost,which could substitute regular western medicine therapy.Improvement of clinical symptoms and life quality,recurrent rate reducing are expected.This new TCM therapy could provide standardized evaluation criteria and evidence for TCM clinical application in hard diseases,and further,contribute to the setting up of an international standard of RE clinical treatment scheme based on TCM.Methods:62 RE patients with spleen deficiency were identified as cases from clinic and gastroscope center.they were randomly divided into two groups and respectively treated with Jianpi Jiangni Recipe(a TCM recipe for descending adversely risen qi to invigorate spleen)and omeprazole for 8 weeks.We observed the changes of clinical symptoms and signs before and after treatment and recorded in the relevant data scales.The effective rate of the two groups was calculated and compared with each other.Clinical effect and advantage of Jianpi Jiangni Recipe for the treatment of RE with spleen deficiency type were evaluatedResults:The improvement rate of four main symptoms were analyzed in both groups,including acid regurgitation,vomiting and salivation,hiccup of belching and epigastric pain.The curative effect was judged by Nimodipine scoring method.In the total of 32 cases of the JPJN group,27 cases were effective and 4 cases were regarded ineffective.In the PPI group,9 cases were cured,22 cases were ineffective.The effective rate was 87.10%in JPJN group compared with 29.03%in PPI group.The improvement rates of the Chinese and western medicine groups under gastroscope were statistically analyzed,it showed the results of both groups were similar.After withdrawal for 3 months,the same tests were carried out again.The total number of effective cases in the JPJN group was 17 cases and 14 cases were ineffective.In the PPI group,7 cases were cured and 24 cases were ineffective.The effective rate was 54.84%in the JPJN group and 22.58%in the PPI group.Conclusion:?The Jiangpi jiangni recipe is effective in relieving symptoms of spleen deficiency type of RE such as acid regurgitation and heartburn,etc.,which is similar to omeprazole treatment.? Jiangpi jiangni treatment has the similar effect in terms of gastroscopic change,comparing to omeprazole treatment.?The Jiangpi jiangni recipe is remarkably more effective than omeprazole in main symptoms,as well as a better long-term effectiveness.? The effectiveness to RE of spleen deficiency type of Jianpi Jiangni Recipe is superior to omeprazole in the secondary symptoms,as is also witnessed in the long term.
