| At present,the definition of social pension service system in most China's policy texts is mainly composed of three parts,home care,community care and institutional care.In the context of the aging,the demand changes of the elderly in China put forward higher requirements for the supply of old-age services,and the contradiction between supply and demand of old-age services is prominent.From the demand side,the old-age service has the characteristics of multi-level and integration.Considering from the supply side,there is also the practice of integration development.The separation and lack of interaction among the old-aged subjects in the community,home and institution lead to the separation and idleness of the old-age resources,which is not conducive to the efficient supply of old-age services.Throughout the development process of social pension service system in various countries and the changing trend of pension service policy in China,we can find that the integration of home-community-institution development is the direction of supply-side reform of pension service and innovation of pension model.The integration and development of old-age services is conducive to improving the effective use of old-age service resources,improving structural contradictions between supply and demand,and enhancing the ability to cope with deep aging.As a pilot city for comprehensive reform of the aged care service industry in Nanjing,Nanjing has already witnessed the of the integration phenomenon of the home-community-institutional aged care services.According to estimates made by the civil affairs department,more than half of the institutions in Nanjing have tried to integrate the development of old-age services.Based on the investigation of the symposium,this paper selects the Jiuru Qinghe North Park Care Service Center in Nanjing as a study case and finds that the current extension from the institution to the community is the mainstream development model of the old-age service integration.Based on Hong Kong's experience in the integration and development of old-age services,this study proposes that the integration of old-age services is the mutual coupling and embedding of home-based endowment resources between institutions and communities.Its main characteristics include demand-based and specialization,system integration,continuous care and service integration,and resource sinking.It also puts forward the integration development paths of institution-oriented,community-oriented and multi-cooperation-oriented,and makes innovations in the mode and way of integration development of China's pension services.From the ideal model to reflect the current challenges in the development of the integration of old-age services,we can find that there are still many obstacles to the institutional mechanism.The separation of the management system,the unclear role of the government in the aged care service,and the imperfect institutional environment have resulted in the relatively isolated operation of the old-age service system,and the low resource integration ability is not compatible with the multi-level service needs of the elderly.This paper proposes some feasible suggestions on how to getting over the obstacles regarding the institutional mechanism on the path of integration and development of old-age services.In the future development of the integration of old-age services,we can carry out integrated management systems,encourage the participation of social forces,promote the construction of pension service standard systems,accelerate the training of endowment service personnel,explore the establishment of long-term care insurance system,and deepen the informationization level of old-age services.The integration of home-community-institutional pension services is not a one-off task,and requires the participation of the government,the old organizations and the public.In terms of institutional reform,we can take in-depth research,practice from pilot to promotion,and move forward gradually. |
